Position Type: Full-time
Schedule: Monday–Thursday (an alternative work schedule may be an option); occasional evenings or on-call hours may be required
Work Location: Office-based with significant field work (home visits and community appointments)

General Statement of Duties:

The Housing Case Manager provides supportive services to individuals and families who are homeless or at risk of homelessness, with the primary goal of achieving and maintaining stable housing. The Case Manager conducts comprehensive needs assessments and develops individualized, housing solution–focused service plans that prioritize immediate housing stabilization while addressing barriers to long-term success. Services include ongoing case management, crisis intervention, advocacy, housing counseling, life and financial skills development, and assistance with employment, education, and income supports. The Case Manager responds promptly to participant crises, utilizes de-escalation and problem-solving strategies, and coordinates with community partners and emergency services as needed to prevent housing loss. All services are delivered using a trauma-informed, strengths-based, and participant-centered approach that supports program goals of housing stability, increased self-sufficiency, and improved overall wellbeing.

Essential Job Functions:

  • Provide housing-focused case management and supportive services
  • Conduct home visits and community-based outreach
  • Transport participants to appointments and housing-related activities
  • Complete assessments, service plans, and progress notes
  • Respond to participant crises and housing stability concerns
  • Coordinate services with community partners and landlords
  • Maintain accurate and timely documentation in HMIS or agency data systems

Qualifications:

A bachelor’s degree in social work, psychology, or a related field is preferred; relevant professional experience and/or lived experience may be considered in lieu of formal education. Experience working with individuals and families who are homeless or at risk of homelessness is desired, as is experience with behavioral health, substance use, crisis intervention, and domestic violence related issues. Candidates must demonstrate strong communication, organizational, and verbal and written skills, as well as strong knowledge of community resources. The ability to work independently and collaboratively as part of a team is required.

The position requires regular local travel, including transporting program participants to appointments, services, and housing-related activities, and conducting home and community-based visits. Candidates must be able to pass a background check and possess a valid driver’s license, reliable transportation, and insurance that meets agency requirements.

Physical & Environmental Requirements:

  • Ability to travel throughout the service area
  • Ability to sit, stand, walk, and move throughout the workday
  • Ability to climb stairs at participant residences and community settings
  • Ability to work in participant homes and other community-based environments
  • Ability to lift, push, and pull up to 20 pounds

Supervisor: Executive Director

Wages: Wages are commensurate with knowledge, experience and skills. Position, hours and wages are dependent upon the availability of grant funding.
