Overview
We are seeking a dedicated and compassionate Case Manager to join our dynamic healthcare team. Responsibilities
- Conduct comprehensive patient assessments to determine individual needs and develop personalized care plans.
- Coordinate with healthcare providers across various departments such as critical care, hospital medicine, emergency medicine, and behavioral health to ensure cohesive patient management.
- Facilitate discharge planning by organizing necessary services, community resources, and follow-up care to support smooth transitions from hospital to home or other facilities.
- Manage patient intake processes, ensuring accurate documentation within electronic health record (EHR) systems while maintaining strict adherence to HIPAA regulations.
- Perform utilization review and management activities to ensure appropriate level of care and resource allocation based on clinical guidelines like ICD-10/ICD-9 coding and CPT coding standards.
- Collaborate with social workers, nurses, physicians, and other healthcare professionals to address complex cases involving hospice care, pediatric needs, or trauma center requirements.
- Maintain detailed case documentation, monitor patient progress, and adjust care plans as needed to meet evolving clinical conditions.
Requirements
- Proven experience in hospital settings with a focus on acute care, ICU experience, or Level I/II trauma centers is highly preferred.
- Strong knowledge of medical terminology, ICD coding (ICD-10/ICD-9), CPT coding, and utilization review processes.
- Background in nursing or social work with experience in behavioral health or managed care is advantageous.
- Familiarity with EMR/EHR systems and discharge planning procedures essential for effective case management.
- Excellent communication skills combined with the ability to coordinate multidisciplinary teams efficiently.
- Ability to handle complex cases involving pediatric patients (including PICU), emergency medicine scenarios, or OR experience is desirable.
- Knowledge of HIPAA compliance standards and patient service principles required for maintaining confidentiality and delivering compassionate care.
