Purpose Of Position:
The Case Manager is responsible for providing case management and support services to adult residents in recovery from drug and alcohol addiction. This position plays a vital role in guiding residents through individualized recovery plans, connecting them to resources, monitoring progress, and ensuring accountability within a Christ-centered, trauma-informed environment.
Specific Position Responsibilities:
- Assure all in- coming addiction recovery intakes in a timely and compassionate manner.
- Conduct and coordinate case assessment meetings for adult residents addressing addiction, trauma, and related needs.
- Develop and monitor individualized recovery plans, ensuring residents move forward in their addiction recovery steps.
- Assign and facilitate life skills, relapse prevention, and recovery workshops for assigned residents.
- Review and respond to Incident Reports, ensuring accountability and appropriate disciplinary action within a grace-based framework.
- Maintain a high standard of confidentiality regarding resident records and case notes.
- Provide clear communication of program requirements and expectations to residents.
- Maintain accurate case records in both hard copy and the ERM/HMIS database.
- Track progress, referrals, vouchers, and compliance with program requirements.
- Assist with database development, program reporting, and outcome statistics.
- Provide referrals to addiction treatment providers, counseling services, employment resources, and other supportive agencies.
- Develop and maintain strong relationships with local treatment centers, mental health providers, and community organizations.
- Participate in agency meetings, service provider collaborations, and community outreach.
- Maintain intake waitlists and ensure prompt communication with potential residents.
- Coordinate weekly orientation meetings for new residents.
- Lead or coordinate educational groups including GED preparation, recovery workshops, and life-skills classes.
- Support program development and improvements through feedback, reporting, and collaboration with leadership.
