Case Manager - Supportive Housing

Job Title: Case Manager - Supportive Housing
Reports to: Program Manager
Location: Boston, MA
Position Type: Full-Time
Hourly Rate: $25.50


Overview:

The Case Manager will work with families who have transitioned from emergency assistance to a pathway to permanent housing by providing stabilization across a two-year program. Working within a team approach to monitor and support efforts of program participants to obtain community resources and services, improve living skills and gain permanent housing.


Responsibilities:

  • Using Motivational Interviewing and a collaborative approach conduct in-depth assessment to best determine the immediate client concerns and/or barriers to housing including, but not limited to: financial, legal, mental health, substance abuse, employment, health care and housekeeping;
  • In collaboration with heads of households, develop individual service plans (ISP) which include educational, vocational, medical, life skills and housing goals. Meet with families in temporary housing provided by Heading Home, weekly to review ISP progress and plan next steps;
  • Obtain resources and services as requested and/or needed to address barriers to housing
  • Provide support and stabilization services to ensure families stay on track for meeting program goals
  • Develop partnerships and collaborative relationships with internal and external service providers to ensure client receives comprehensive services;
  • Assist client with move-in transition supports including but not limited to; furniture and furnishings assistance, budget support, and other resources and services as needed;
  • Develop and implement a housing intake/history with homeless clients;
  • Document all interactions with clients utilizing appropriate reporting mechanisms, including web-based data collection systems;
  • Monitor units for cleanliness, or property damage and address with client as needed;
  • Participates in individual and group supervision as assigned
  • Participates in all required training and professional development activities;
  • Perform other tasks as requested by the Director of Mobility Programs

WORK ENVIRONMENT

Cubicle space within large office building; The office is open to the public by appointment ONLY. The position requires a minimum of four days onsite. The office is climate controlled with heat and air conditioning.

Requirements:

  • BA Degree with one-year case management or related work experience; or any combination of education/work experience that equips applicant to carry out the above listed responsibilities
  • Knowledge and experience with service agencies and resources for homeless individuals and/or families
  • Willingness to accompany clients as needed to appointments
  • Willingness to participate in an on-call rotation
  • Sensitive to the needs of the homeless, low income and diverse populations
  • Ability to work in a busy, diverse team setting
  • Excellent time management, organizational and communication skills
  • Ability to independently travel between client appointments and offices as needed
