Schedule/Hours: Monday - Friday, 8:00 a.m. – 4:00 p.m. (37.50 hours a week)

This position is responsible for:

Provides social service assistance to eligible participants; interviews participants to determine needs and eligibility to receive assistance and/or eligible for the Pathway of Hope or Up and Out programs. Case Worker ensures compliance with program policies and procedures; monitors assigned caseload of participants; prepares accurate and complete participant records; establishes program plan/goals and evaluates participant's progress by conducting routine meetings with them and/or staff; serves as advocate for participants in order to acquire services that will enable them to functionally cope with their environment.

Key Responsibilities:

  • Interviews participants requesting assistance to determine eligibility for assistance and/or the Pathway of Hope or Up and Out program based on established guidelines and requirements; conduct needs assessment, obtain pertinent information as guided by Salvation Army policies and procedures; establish program goals for eligible participants; refers applicants to other agencies if not appropriate for programs.

  • Develops intake and case plans for Pathway of Hope and Up and Out participants; instructs participants in completing necessary paperwork including goals and/or budget forms; completes orientation check-off form and obtains release of information forms when appropriate; completes social history on new participants each week.

  • Prepares and maintains case records and logs on all participants; develops intake and case plans for program participants; instructs program participants in completing necessary paperwork including goals and/or budget forms; completes orientation check off form and obtains releases of information and appropriate forms.

Physical Requirements and Working Conditions:

  • Ability to meet attendance requirements.

  • Ability to read, write, and communicate the English language.

  • Ability to communicate clearly and effectively both orally and in writing.

  • Ability to think clearly and quickly in order to maintain control of client caseload.

  • Ability to effectively and efficiently work on multiple cases at the same time without becoming frustrated or disorganized.

  • Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25lbs) 5-10% of work time.

  • Work is performed in a normal office environment where there are little or not physical discomforts associated with weather or discomforts associated with noise, dirt, and the like. Work requires performance of duties in a social services environment where exposure to communicable and infectious diseases are possible.

  • Two years of college coursework from an accredited college or university in Social Work, Behavioral Science, or a related field, AND

  • Two years’ experience working in a social or public service environment with experience assisting the public OR

  • Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Licenses and Certifications:

  • Valid State Driver’s License
