Case Worker - Homeless Coordinator

Homeless Coordinator will provide outreach and voluntary case management to homeless families and individuals that support the Ogden Housing Authority's Homeless programs. Responsibilities include:

• Administer homeless programs to insure program rules and guidelines are followed and funding is maximized;
• Provide outreach and initiate contact with potential clients;
• Ensure clients understand program expectations and regulations;
• Provide supportive services to clients as needed to include housing search and maintaining housing; contacting prospective landlords and/or management companies, accompanying clients to view and apply for units, attending appointments at housing agencies and other search activities as they present; intervene as needed on the clients behalf to insure stable housing.
• Responsible to collaborate and gather input with the person or household experiencing homelessness to identify their current needs and implement a supportive service plan immediate and ongoing needs. Ensure client input to help creating goals to include other goals that include employment needs, mental and physical health needs, transportation, crisis needs, and so forth.
• Educate clients about community resources and increase awareness of and support client participation in consumer self-help and advocacy organizations.
• Participate and present homeless data at local homeless committee and other necessary meetings. Participate in annual homeless point in time count.
• Attend community meetings as needed to enhance the work of the outreach team and/or continuum of homeless services
• Make home visits to clients who have obtained permanent housing as needed and/or requested.

MINIMUM REQUIREMENTS: Associates degree in social work or two (2) years equivalent work experience in social work in related field if not degreed. Computer literate and ability to communicate effectively both verbally and in writing. Must enjoy being around and helping others. Demonstrated proactive stance in case management.
