Overview
We are seeking a compassionate and dedicated Caseworker to join our team, providing essential support and guidance to individuals and families facing various challenges. Duties

  • Conduct assessments to identify client needs related to child welfare, addiction, mental health, and housing stability
  • Provide crisis intervention and management to clients experiencing urgent or complex situations
  • Offer addiction counseling and behavioral health support tailored to individual circumstances
  • Assist clients with navigating social services such as Section 8 housing, Medicare, and workforce development programs
  • Implement motivational interviewing techniques to encourage positive behavioral changes
  • Supervise and mentor clients, including those involved in childcare or mentoring programs
  • Coordinate with community agencies and service providers to ensure comprehensive care and resource access
  • Document case progress, develop individualized plans, and maintain accurate client records in compliance with agency policies

Requirements

  • Proven experience in social work, crisis management, or related fields such as behavioral health or child welfare
  • Strong skills in crisis intervention, addiction counseling, and motivational interviewing
  • Supervising experience or background working with children or mentoring programs is preferred
  • Knowledge of community resources including Section 8 housing, Medicare, and workforce development initiatives
  • Excellent communication skills with the ability to manage sensitive situations professionally
  • Ability to handle multiple cases effectively while maintaining attention to detail
  • Compassionate attitude with a commitment to client-centered service delivery
  • Relevant educational background or certifications in social work or related disciplines is desirable

Job Location: 928 Coney Island Avenue, Brooklyn NY 11230 (City Sanctuary Facility)

Starting pay : $40,000 (Based on Experience)

Bilingual: English, Spanish, Russian, Turkish and or Persian

Rotating Shifts: must be able to work an evening shift and weekend

Job Description:

People’s Path Inc. is a Non-Profit Organization; we are currently looking for Case Workers that will be

responsible for providing comprehensive case management services to our clients in need. This includes

assessing clients needs, developing individualized service plans, and coordinating with various

resources and services to help our clients achieve their goals. The Case Workers will work closely with

clients, their families, and other service providers to ensure our clients wellbeing and care.

Education, Certificates or Credentials:

Bachelor’s degree in Social Work, Phycology, Sociology, Human Services or a related field. NYS

Driver’s License –(Required)

Case Worker Skills and Qualifications:

Client Documentation, Analyzing Information, Decision Making, Research Skills, Verbal

Communication, People Skills, Resolving Conflict, People Management, Integrity, Able to connect with

children. Proficiency in case management software and Microsoft Office products. Ability to work

independently and as part of a team.

Work Experience Requirements:

Minimum Two (2) years of customer service or case worker, social work experience, preferably

working with the undeserved population, including but not limited to, homelessness, substance use,

mental health, and LGBTQ population with a clear demonstrated commitment to client customer

service.

Job Responsibilities:

Client Assessment: Conduct thorough assessments of clients’ needs, strengths, and goals through

interviews, observations, and review of records.

Service Planning: Develop individualized service plans in collaboration with clients and other relevant

parties. Set achievable and measurable goals.

Resource Coordination: Identify and connect clients with appropriate resources, services, and

support systems. This includes housing, healthcare, employment, education, and financial assistance.

Advocacy: Advocate for clients’ rights and needs within the organization and in the broader

community. Assist clients in navigating complex systems.

Case Management: Provide ongoing case management services, including regular follow-up

meetings, progress tracking, and adjustments to service plans as needed.

Documentation: Maintain accurate and up-to-date records of client interactions, service plans, and

progress reports. Ensure confidentiality and compliance with organizational and legal standards.

Crisis Intervention: Provide immediate support and intervention in crisis situations. Develop and

implement safety plans as necessary.

Collaboration: Work collaboratively with other team members, volunteers, and external partners to

ensure comprehensive service delivery.

Training and Development: Participate in training and professional development opportunities to

stay informed about best practices in case management and social services.
