Job Description:

Company Overview: SHA Diabetes Center (SDC) LLC is a leading healthcare provider dedicated to delivering high-quality patient care in Diabetes. We are seeking a Certified Registered Nurse Practitioner (CRNP) and Certified Physician Assistant (PA-C) to join our dynamic healthcare team. Insulin Pumps, CGM devices and wound care experience are a plus, but we welcome all qualified candidates who are committed to delivering exceptional healthcare services.

Position Overview: As a Certified Registered Nurse Practitioner (CRNP) or Certified Physician Assistant (PA-C) at SDC LLC, you will play a crucial role in providing comprehensive healthcare services to our patients. You will work collaboratively with our team of healthcare professionals to deliver patient-centered care, focusing on preventive, acute, and chronic healthcare management. The ideal candidate will have experience with Insulin Pumps, CGM devices, and wound care experience, but we are also open to those willing to learn and expand their expertise in these areas.

Responsibilities:

• Perform comprehensive patient assessments, including medical history reviews and physical examinations.
• Diagnose and treat a wide range of medical conditions and illnesses related to Diabetes.
• Develop and implement patient care plans, including ordering and interpreting diagnostic tests.
• Prescribe medications and treatments, as necessary.
• Educate patients and their families on healthcare maintenance and disease prevention.
• Collaborate with other healthcare professionals to coordinate patient care.
• Provide wound care services, including assessment, debridement, and dressing changes.
• Manage patients with Continuous Glucose Monitoring (CGM) devices, including interpreting data and adjusting treatment plans (experience preferred but not required).
• Manage patients with Insulin Pumps and adjust appropriately as needed.
• Maintain accurate and up-to-date patient records.

Qualifications:

• Current CRNP or PA certification and state licensure.
• Graduation from an accredited Nurse Practitioner or PA program.
• Strong clinical assessment and diagnostic skills.
• Ability to work collaboratively with a healthcare team.
• Excellent communication and interpersonal skills.
• CGM, Insulin Pump, and wound care experience is a plus, but not required.
• Commitment to continuing education and professional development.

Hours of Operation: Monday – Friday: 8 AM – 5 PM. Saturday & Sunday: Off, No Calls
