Clinical Case Manager- Float (Weekend Coverage Required)

JOB SUMMARY

Reporting to the Clinical Director, the Clinical Case Manager at Neurish Wellness plays a crucial role in our residential mental health treatment outcomes. Our interdisciplinary psychiatric team prioritizes the clients' needs and involves them in the treatment planning process. As a cornerstone of our clients' success, the Clinical Case Manager (or CCM) shares our passion for enriching and restoring lives. The CCM will have an immediate impact on our clients’ lives, facilitating excellent client care, helping with improved diagnostics, and improving client outcomes. The CCM must have excellent clinical acumen and succeed in delivering quality care, comfort, and safety that surpasses industry standards in line with our mission, vision, and values.

  • The CCM (Float) is needed for weekday and weekend coverage, as they will work at all 4 locations as directed by the Clinical Director.

Neurish Wellness operates a series of evidence-based treatment centers for adults struggling with mental health diagnosis & co-occurring issues. At Neurish Wellness, we offer clinically sophisticated mental health treatment for patients suffering from mental illness to restore & change the quality of their lives. Below are some, but not all that is expected as general duties and responsibilities:

  • Assessing clients’ physical and mental wellness, needs, preferences, and abilities while using these to develop treatment plans through collaboration with the interdisciplinary team.
  • Taking ownership over the client's discharge & aftercare plans informed by what is in the best interest of the client and best practice for the industry.
  • Continuing to tailor these discharge & aftercare plans to enhance chances of successful completion and improved outcomes for our clients.
  • Coordinate and manage client transfers to higher or lower levels of care and discharges against clinical advice (ACA blocking & intervention a requirement).
  • Work systemically with clients, family and friend support networks, and health care professionals to continuously evaluate if treatment plans need to be adjusted or corrected.
  • Study client behavior, listen to clients’ needs, and utilize all information to inform and provide counseling or intervention as required.
  • Evaluate clients’ progress periodically and adjusting it as needed to improve outcomes.
  • Following up with discharged clients (in coordination with Alumni services) to ensure they were satisfied with services, and they are still in good physical and mental health.
  • Lead and provide group therapy sessions utilizing Evidence Based Practices (EBP) & psychoeducation.
  • Prepare clinical documentation accurately according to all accreditation requirements and within agency time frames.
  • Respond appropriately to different situations common among dual diagnosis and mental health disorders, including those with behavioral and developmental needs.
  • Intervene during crisis intervention situations.
  • Provide behavioral interventions and communication tools to provide effective feedback to patients.
  • Adhere to code of ethics, legal aspects of clinical practice, professional standards, duty to warn, abuse and neglect reporting policies and procedures, and exercise professionalism always.
  • Effectively assist patients in advocating for their personal needs.

Education, Experience, Skills, & Abilities

  • Education: bachelor’s degree in psychology or social work, or in an appropriate field of study.
  • Master’s degree and licensed by Board of Behavioral Sciences is preferred (Not required).
  • 3-5 years of experience in a behavioral healthcare field in a residential or hospital setting is preferred.
  • Active CADC-I, CADC-II, MSW, ACSW, or like credential is preferred.
  • Experience working with a population that has substance use disorder (SUD) and mental health disorders.
  • Preferred experience working with patients suffering from Severe Mental Illness-(SMI)
  • Possess a valid driver’s license.
  • CPR/First Aid Certification
  • Working knowledge of Microsoft Word, Excel, and Outlook.
  • Must pass FBI/DOJ Livescan and Background check

PHYSICAL REQUIREMENTS

· Work environment is indoors and climate-controlled. Occasionally exposed to outdoor weather conditions. Moderate noise levels as found in a business office with computer printers, households with TVs and dishwashers, and driving light traffic.

Job Type: Full-time

Pay: $28.00 - $32.00 per hour
