Clinical Pharmacy Manager

Job Summary and Qualifications

The Clinical Pharmacy Manager is responsible for designing, implementing and managing the pharmacy clinical programs. This includes formulary management, policy and procedure development and education across the healthcare spectrum. The Clinical Manager shall also assist in managing the pharmacy when the Pharmacy Director or designee is unavailable.

What you will do in this role:

  • Interprets orders and transcribes or verify them accurately and efficiently into the pharmacy computer system
  • Accurately compounds and dispenses pharmaceuticals
  • Comply with facility policy and procedures and regulatory requirements
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
  • Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed
  • Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy selecting
  • Supports participation in daily multidisciplinary and bedside rounds (may need to attend)
  • Identifies, designs and analyzes drug utilization reviews to promote rational drug therapy and determines action plan based on results
  • Provides accurate and timely medication information to healthcare staff and patients
  • Develops, manages and mentors pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as determined appropriate based on patient population and literature guidelines.
  • Develops, manages and mentors for the antimicrobial management program (AMP)
  • Documents all clinical activities and interventions accurately and completely
  • Responsible for mentoring and education as well as ongoing analysis of interventions to ensure accurate and appropriate documentation and provides summary reports
  • Supports and participates in the goals of the organization’s quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives
  • Is responsible for design, analysis and reporting of adverse drug reactions, medication errors and pharmacy quality initiatives and mentors others in their roles in these programs
  • Develops and actively participates in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses and physicians and maintains responsibility for assessment of participants in these programs
  • Develops and documents staff competency on an ongoing basis and develops improvement plans when deficiencies in competencies are identified
  • Primary preceptor for pharmacy students and/or residents as it relates to clinical pharmacy services
  • Responsible for designing and managing programs and mentoring student/resident as well as other preceptors.
  • Maintains professional competency sufficient to meet current and future needs of the facility.
  • Develops and manages specialty pharmacy practice programs related to clinical pharmacy.
