A Brief Overview
Stormont-Vail HealthCare PGY1 Pharmacy Residency Program provides comprehensive pharmaceutical care to patients through optimizing medication therapy and promotes health, wellness and disease state prevention under the guidance of Clinical Pharmacy Specialists. SVHC PGY1 Pharmacy Residents will uphold American Health-Systems Pharmacists and American College of Clinical Pharmacist philosophy of pharmaceutical care; which is blending a caring orientation with specialized therapeutic knowledge, experience and judgment for the purpose of ensuring optimal patient outcomes.

Education Qualifications

  • Bachelor's Degree Bachelor's of Science in Pharmacy Required or
  • Doctorate PharmD Required


Skills and Abilities

  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Excellent customer service skills. (Required proficiency)
  • Medical terminology and knowledge of medication names. Spreadsheets, Internet/E-mail, Database software, Contact Management, Electronic Medical Record. (Required proficiency)
  • Interpersonal skills to effectively communicate and interact with all members of the healthcare team while multitasking to ensure all assigned tasks are completed. (Required proficiency)
  • Ability to apply advanced pharmaceutical mathematical concepts including but not limited to the following: pharmacokinetic calculations, Child Pugh score, Cockcroft- Gault calculations, and other pertinent calculations based on patient population. (Required proficiency)
  • Ability to define problems, collects data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. (Required proficiency)


Licenses and Certifications

  • Pharmacist - KBP Licensed pharmacist in the State of Kansas OR must apply for Kansas Licensure within 30 days of hire date and to obtain pharmacist licensure within 120 days of the hire date. Required


What you will do

  • Participate in patient care activities including review labs, microbiology, vitals signs, MAR to assure medication therapy is appropriate or if additional intervention. Participate in drug therapy management of medical emergencies (code, trauma, stroke, snake bite etc.). Recommend appropriate pain management therapy when requested. Provide pharmacokinetic consultations and other dosing as required.
  • Review medication orders for accuracy and appropriateness. Discuss medication order clarification with the prescriber, document any changes or interventions as per policies, and communicate with others about medication order changes if needed. Anticipate changes in drug therapy and report the potential impact it will represent to the overall hospital and drug budget.
  • Prevent, detect, monitor, document and report adverse drug reactions and medication errors. Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
  • Help to identify and implement drug therapy where positive pharmacoeconomic impact could be achieved. The ultimate goal is to develop and assure cost effective clinical pathways. Promote and demonstrate rational, cost-effective drug therapy. Conduct target drug programs and medication use evaluations as needed, and report results to the Pharmacy and Therapeutics Committee or subcommittees. Active participant in SVH pharmacy committees and related such as Antibiotics Stewardship and Opioid Stewardship committees.
  • Participate in clinical research, performance improvement projects. Presenting research and/or provide education at a professional conferences. Provide presentations, publication and other informative activities pertaining to designated patient care area and other drug-related topics to the health care community and general public.
  • Work closely with preceptors, coordinators and program director and follow the required learning experience (rotations) and longitudinal activities. complete all classes, courses and required tasks and certificates within the timeline approved by preceptors, coordinators or residency director.
  • Develop leadership and practice management skills within and outside of the employing institution
  • Counsel/triage patients on discharge/transfer medications as necessary.
  • Assess insurance drug coverage in advance of discharge/transfer and assist in preauthorization for medications if necessary.
  • Provide operational support in the designated patient care department and central pharmacy as determined by departmental needs.
  • Coordinate with technician to resolve drug distribution related issues. Coordinate with technician to resolve Omnicell related issues (i.e. discrepancies, access issues, etc).
  • Coordinate with technician to assure patient care units comply with JCAHO medication standards. Follow JCAHO medication compliance standards.
  • Take a leadership role and represent the pharmacy department in designated patient care services units by attending department meetings, assuring performance improvement and other quality measures are met and educating staff regarding the safe and appropriate use of medications.
