Role Overview

Our valued partner is seeking a Clinical Readiness Specialist to support clinical teams in delivering high-quality, efficient, and well-coordinated patient care through strong pre-visit preparation, care gap identification, and population health support.


This role is focused on ensuring that patient visits are fully prepared in advance by reviewing charts, identifying missing clinical information, and supporting outreach efforts that improve preventive care completion and chronic care management.

The successful candidate will work closely with providers, care coordination teams, and scheduling staff to improve visit efficiency, close care gaps, and enhance continuity of care across the patient population.


This position is fully remote and operates as a non-clinical support role, meaning no hands-on patient care is involved. Instead, it plays a critical operational and population health function within the clinical ecosystem.


Key Responsibilities

Clinical Chart Preparation & Visit Readiness

  • Review upcoming provider schedules to ensure patient charts are complete, accurate, and ready prior to appointments.
  • Identify care gaps including preventive services, chronic condition monitoring, and health maintenance requirements.
  • Review prior labs, referrals, imaging, and clinical documentation to ensure continuity of care.
  • Flag missing or incomplete clinical information and route it to appropriate clinical teams for resolution.
  • Support the preparation of efficient clinical workflows that improve provider productivity and patient experience.

Patient Outreach & Care Gap Closure

  • Conduct outbound patient outreach to support completion of preventive care services, screenings, and follow-ups.
  • Encourage scheduling of annual wellness visits, chronic care appointments, labs, imaging, and referrals.
  • Reinforce patient engagement around preventive health and continuity of care.
  • Document outreach activities and outcomes in electronic systems and tracking tools.
  • Coordinate with scheduling and care teams to ensure timely follow-up actions are completed.

Care Coordination & Workflow Support

  • Support care coordination teams in tracking outstanding patient care needs and follow-up requirements.
  • Assist in closing referral loops and ensuring completion of provider-ordered diagnostics and services.
  • Maintain tracking systems to monitor patient progress toward care gap closure and visit readiness.
  • Act as a liaison between clinical, operational, and scheduling teams to ensure smooth patient flow.

Data Management & Quality Reporting

  • Perform accurate data entry and documentation within electronic health records and clinical systems.
  • Maintain care gap tracking tools, outreach logs, and follow-up records.
  • Support reporting on outreach activity, care gap closure, and population health metrics.
  • Assist in quality improvement initiatives focused on access, preventive care, and clinical performance.
  • Contribute to audit readiness including HRSA, Joint Commission, and other regulatory reviews.

Compliance & Organizational Support

  • Maintain strict compliance with HIPAA, privacy, and confidentiality requirements.
  • Follow organizational policies, procedures, and clinical governance standards.
  • Participate in Patient-Centered Medical Home (PCMH) activities and training programs.
  • Engage in ongoing learning, meetings, and professional development activities.
  • Support the mission of improving equitable access to high-quality healthcare.


Requirements

Experience & Education

  • High School Diploma or GED required.
  • Minimum 1 year of experience in healthcare settings such as medical assisting, care coordination, patient outreach, population health, or community health.
  • Experience in FQHC or PCMH environments is strongly preferred.

Technical & Functional Skills

  • Experience working with electronic health records (NextGen preferred).
  • Strong ability to manage documentation, tracking tools, and clinical data workflows.
  • Familiarity with care gap closure, preventive care measures, or population health support is preferred.
  • Ability to work independently in a remote, productivity-driven environment.

Communication & Interpersonal Skills

  • Strong written and verbal communication skills.
  • Ability to coordinate effectively with clinical, operational, and administrative teams.
  • Comfortable engaging patients in structured outreach conversations.
  • Strong interpersonal skills with a patient-centered approach.

  • High attention to detail and strong organizational skills.
  • Ability to manage multiple workflows and deadlines simultaneously.
  • Analytical thinking with the ability to interpret clinical and operational data.

Additional Preferences

  • Medical Assistant training or certification (non-clinical role).
  • Experience supporting quality measures or healthcare performance metrics.
  • Bilingual English/Spanish is strongly preferred.
