Job Summary

Duties
- Monitor and observe patients’ conditions closely, recording vital signs and changes in health status using Electronic Health Record (EHR) or EMR systems.
- Assist with Activities of Daily Living such as bathing, dressing, grooming, mobility support with Hoyer lifts, and toileting including catheter care.
- Support patients with medication administration following medical documentation protocols while ensuring HIPAA compliance to protect patient privacy.
- Provide specialized dementia and Alzheimer’s care, including memory care techniques to ensure safety and comfort.
- Help with meal preparation, feeding, cleaning, laundry, and maintaining a clean and safe environment for residents or clients.
- Support hospice and palliative care needs by offering compassionate emotional support alongside medical assistance.
- Use medical terminology accurately when documenting patient care activities and communicating with healthcare teams.
Requirements
- Valid certification as a CNA, HHA, or Caregiver with current licensing where applicable.
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred.
- Demonstrated patient observation skills along with knowledge of medical terminology and vital signs monitoring.
- Experience working with individuals with developmental disabilities or dementia is advantageous.
- Certifications in CPR (Cardiopulmonary Resuscitation), hospice care training, and HIPAA compliance are required or preferred.
- Ability to operate EMR/EHR systems efficiently for medical documentation and records management.
