Overview:
The Caregiver II provides compassionate, hands-on personal care and support to patients receiving home care and/or center-based services. Under the direction and supervision of a Supervisor or Manager, this role supports patient dignity, safety, and independence while ensuring care is delivered according to individualized care plans. In addition to direct care, the Caregiver II plays a leadership role by supporting onboarding and ongoing training of fellow caregivers. This position is ideal for an experienced caregiver who enjoys mentoring others and delivering high-quality, patient-centered care in a collaborative healthcare environment.

What you will be doing:

  • Provide personal care and escort assistance in accordance with individualized care plans, including: Grooming, dressing, peri-care, bathing, showering, and toileting, Housekeeping, laundry, meal preparation, feeding assistance, etc.

  • Observe and report changes in patient condition to the team in a timely and professional manner.

  • Demonstrate safe care practices by using proper body mechanics, adhering to universal precautions, and maintaining a safe environment.

  • Interact professionally with patients, families, and team members, encouraging engagement in conversation and activities.

  • Perform all duties of a Caregiver I as needed.

  • Support onboarding and ongoing training of Caregiver I and Caregiver II staff using established training materials and manuals.

  • Actively participate in departmental meetings and family meetings as required.

  • Utilize safety equipment appropriately for lifting and transfers; provide positive guidance to others on proper use.

  • Perform other related duties as assigned.


What you must have:

Experience

  • Minimum of one (1) year of experience working with elderly or frail populations.

  • Experience working with seniors with dementia.

Knowledge, Skills & Abilities

  • Understanding of an integrated, patient-centered care model gained through experience or training.

  • Flexible, positive attitude with the ability to work across multiple care settings.

  • Ability to work independently and collaboratively as part of a team.

  • Strong interpersonal and communication skills.

  • Basic English reading and writing skills required; bilingual ability may be required for specific assignments and daily patient interaction.

  • Basic computer skills, including the ability to use or learn email and electronic health record systems.

  • Ability to explain and model safe lifting techniques and proper use of safety equipment.

Licenses & Certifications (Required Upon Hire)

  • Home Health Aide (HHA) or Certified Nursing Assistant (CNA)

  • CPR (Cardiopulmonary Resuscitation)

  • First Aid
