Responsibilities

• Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patients’ health by taking vital signs, observing behavior, and reporting changes promptly
• Administer medications following care plans and ensure compliance with HIPAA regulations for patient privacy
• Prepare nutritious meals and assist with cooking to meet dietary needs and preferences
• Support individuals during community outings, appointments, and social activities to promote engagement and independence
• Maintain a clean and safe environment through regular cleaning tasks and adherence to safety protocols
• Use specialized equipment such as Hoyer lifts or resident’s <PERSON> to ensure safe transfers and mobility assistance
• Document care activities accurately in compliance with healthcare standards and organizational policies
• Collaborate with healthcare professionals, family members, and team members to develop and follow personalized care plans
• Provide emotional support, companionship, and encouragement to foster a trusting relationship

Skills

• Proven experience working in assisted living, group home settings, or long-term care environments
• Knowledge of patient care techniques including CPR, first aid, dementia care, Alzheimer’s care, memory care, hospice care, autism support, and working with individuals with disabilities
• Strong observational skills for patient monitoring and patient safety oversight
• Ability to perform heavy lifting safely when required for resident transfers or mobility assistance
• Excellent communication skills to effectively interact with residents, families, and healthcare teams
• Familiarity with HIPAA regulations ensuring confidentiality of patient information
• Experience with medication administration and care plan implementation
• Capable of managing multiple responsibilities in a fast-paced environment while maintaining attention to detail
• Compassionate attitude combined with patience and resilience in caring for individuals facing complex health challenges
