Responsibilities
- Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health by taking vital signs, observing behavior, and reporting changes promptly
- Administer medications following care plans and ensure compliance with HIPAA regulations for patient privacy
- Prepare nutritious meals and assist with cooking to meet dietary needs and preferences
- Support individuals during community outings, appointments, and social activities to promote engagement and independence
- Maintain a clean and safe environment through regular cleaning tasks and adherence to safety protocols
- Use specialized equipment such as Hoyer lifts or resident’s <PERSON> to ensure safe transfers and mobility assistance
- Document care activities accurately in compliance with healthcare standards and organizational policies
- Collaborate with healthcare professionals, family members, and team members to develop and follow personalized care plans
- Provide emotional support, companionship, and encouragement to foster a trusting relationship
Skills
- Proven experience working in assisted living, group home settings, or long-term care environments
- Knowledge of patient care techniques including CPR, first aid, dementia care, Alzheimer’s care, memory care, hospice care, autism support, and working with individuals with disabilities
- Strong observational skills for patient monitoring and patient safety oversight
- Ability to perform heavy lifting safely when required for resident transfers or mobility assistance
- Excellent communication skills to effectively interact with residents, families, and healthcare teams
- Familiarity with HIPAA regulations ensuring confidentiality of patient information
- Experience with medication administration and care plan implementation
- Capable of managing multiple responsibilities in a fast-paced environment while maintaining attention to detail
- Compassionate attitude combined with patience and resilience in caring for individuals facing complex health challenges
