At North Lake Care Center, we are seeking an experienced Director of Rehab / Physical Therapist to join our team of dedicated healthcare professionals. As a Director of Rehab, you will be responsible for overseeing the rehabilitation department and providing high-quality physical therapy services to our residents. Responsibilities:

• Oversee the daily operations of the rehabilitation department, ensuring compliance with state and federal regulations

• Provide physical therapy services to residents, including initial assessments, treatment planning, and implementation of intervention plans

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Supervise and mentor rehab team members to ensure exceptional patient care and outcomes

• Conduct monthly quality assurance reviews and implement performance improvement initiatives

• Develop and maintain relationships with referring healthcare providers and community partners

• Participate in facility-wide committee meetings and contribute to QAPI initiatives

Requirements:

• Master's degree in Physical Therapy (PT) from an accredited program

• Current licensure to practice as a Physical Therapist in the state of Florida

• Minimum of 2 years of experience in a rehabilitation setting as a Physical Therapist, with experience in a leadership role.

• Strong knowledge of Medicare and Medicaid regulations and guidelines

• Excellent communication and interpersonal skills

• Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
