Job Overview
We are seeking a compassionate and dedicated DSW/Caregiver to join our dynamic team, providing essential support and care to individuals in various care settings. Duties

  • Assist residents or clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
  • Monitor patient health by checking vital signs and observing for any changes in condition
  • Support medication administration following HIPAA regulations and care plans
  • Prepare nutritious meals and assist with meal planning to meet individual dietary needs
  • Operate equipment such as Hoyer lifts and ensure safe patient transfers while adhering to safety protocols
  • Maintain a clean and safe environment through regular cleaning and sanitation tasks
  • Document patient observations, care activities, and incident reports accurately and promptly
  • Collaborate with nursing staff, social workers, and other healthcare professionals to develop and implement personalized care plans
  • Provide companionship and emotional support to promote mental well-being
  • Follow all safety procedures including CPR, First Aid, and emergency response protocols

Skills

  • Proven experience in assisted living, home health, or long-term care environments
  • Knowledge of working with individuals with developmental disabilities, autism, Alzheimer's disease, dementia, or memory care needs
  • Strong patient observation skills to detect changes in condition or behavior
  • Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
  • Familiarity with HIPAA regulations ensuring confidentiality of patient information
  • Excellent communication skills for effective interaction with residents and team members
  • Ability to work independently as well as part of a collaborative caregiving team
  • Experience in medication administration and vital signs monitoring is highly desirable
  • Compassionate attitude with a genuine desire to improve the quality of life for others
