Job Overview
We are seeking a dynamic and compassionate Family Nurse Practitioner to join our dedicated healthcare team. In this role, you will deliver high-quality primary care services, perform assessments, diagnose illnesses, and develop treatment plans for diverse patient populations. You will also participate in the care of our Rheumatology patients in various situations. Your expertise will help improve health outcomes and enhance patient experiences across outpatient settings.

Duties

  • Conduct thorough physical examinations and patient assessments to determine health status.
  • Manage acute and chronic conditions, including urgent care needs and long-term disease management.
  • Prescribe medications, including injections and IV infusions, following clinical guidelines and safety protocols.
  • Ability to place an IV or venipuncture
  • Utilize EMR (Electronic Medical Records) systems like Epic, eClinicalWorks, or Cerner to document patient encounters accurately. Our system is Greenway PrimeSuite.
  • Collaborate with multidisciplinary teams on discharge planning, case management, and utilization review to optimize patient care.
  • Engage in telehealth consultations to expand access to care and support remote patient management.

Skills

  • Strong clinical skills in areas such as Adult Primary Care and Acute Illness such as URI, UTI, etc.
  • Expertise in performing procedures including venipuncture, IV placement, catheter care, and sterile techniques.
  • Knowledge of CPT coding, ICD-10/ICD-9 coding systems for accurate documentation and billing.
  • Familiarity with occupational health protocols, hospice & palliative medicine practices, and post-acute care processes.
  • Excellent patient communication skills with a focus on health coaching and education for individuals of all ages—including children and seniors with complex needs.
  • Working knowledge of HIPAA compliance standards to ensure confidentiality and secure handling of medical records. Join us to make a meaningful difference in patients’ lives through innovative healthcare delivery!

This position will be a part time to full time conversion position. We will increase hours as your practice builds its patient base. We offer continuing education in both Primary Care and Rheumatology. Experience is preferred but not required for this position. New Graduates are highly encouraged to apply.

Pay: $50.00 - $65.00 per hour

Expected hours: 24.0 – 36.0 per week
