As a Bi-Cultural Community Case-Manager you will provide one-on-one navigation services to individuals and families to help discern their needs and give them county resources as a part of our Bi-Cultural Service Navigation Program. Provides linkage support including linguistic and cultural interpretation of documents and services. You will report directly to the Program Director and Executive Director. You will also work with other Youth and Family Center programs to ensure quality programming for our community. See below for more information:
Background & Experience
- Required Bilingual in English and Spanish
- Required Associate's Degree, Bachelor's Degree Preferred and/or;
- Five years of experience working with a social serving agency, specifically with youth and families preferred
- Background and/or experience with working with immigrants or family of immigrant communities
Schedule
- 40 hours a week. Flexible Schedule.
- $18.00/hr-$20/hr based on experience and education
- Some weekends may be required
- Standard office hours are 9:00 a.m. to 5:00 p.m., with flexibility to work evening hours until 7:00 p.m. as needed based on program and client needs.
Job Responsibilities & Expectations
- Ability to initiate, organize, and implement all aspects of the Navigation Service Program core elements which includes, but are not limited to the following
- Provide outreach, needs assessment, referrals, intake, and support to individuals and families
- Meet with clients one-on-one to asses needed services and support
- Provide linkage support, facilitating communication between mental health, substance abuse, education, and other community institutions
- Providing linguistic and cultural interpretation of documents and services
- Coordinate with other county service providers to ensure client access to services
- Document case management progress notes and intake forms
- Report program updates, complete monthly reports
- Ability to deliver services in a variety of community settings
- Keep open communication with Director regarding the Navigation Program
- Maintain a safe and healthy environment within the program
- Assist in maintaining community partnerships
- Remain flexible to the needs of the program and organization
