As the Harm Reduction Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:

  • Conduct outreach activities to engage the target population
  • Identify community partners to engage PWUD who are unaware of their status or untreated
  • Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks
  • Provide enrollment into the agencies Syringe Exchange Program
  • Facilitate brief assessments with the target populations
  • Through review of the assessment, identify community based services to provide client linkage
  • Facilitate linkage to services through escort as necessary


Harm Reduction Case Manager
Minimum Salary: $42,000

Harm Reduction Case Manager Maximum Salary: $42,000

Harm Reduction Case Manager Schedule: Mon – Fri 8:30am – 4:30pm


Qualifications:

  • High School Diploma or GED
  • 1-3 years of relevant experience, Human Services
  • Must have valid Driver's License and access to reliable vehicle
