Adult & Aging Services Home and Community Based Services is seeking a HCBS Case Manager in Minot, ND
The ND Department of Health & Human Services (HHS) Adult & Aging Services Division is seeking a Home and Community Based Services Case Manager to assist individuals to be able to remain in their own home with supportive services. This position is a Telecommute position and will be assisting individuals in the Location and surrounding area.
As our HCBS Case Manager, you will provide case management to elderly and physically disabled individuals under the Home and Community Based Services Program. You will perform assessments, develop, and implement individualized person-centered care plans, and provide monitoring and on-going case management. Additionally, you will make referrals to community partners and maintain records and documentation.
To thrive in this position, you must be proficient in use of a computer, as well as possess good time management and organizations skills. This position requires the ability to visit clients in their own homes or in facility settings and will involve travel to be able to conduct on-site assessments with clients.
If no applicants meet the above qualifications, consideration will be given to applicants who have passed their ND Social Work licensure exam and are awaiting their license number. Consideration will also be given to candidates who have their LBSW and less than one year of experience.
Location
ND HHS utilizes a blended workplace model. Depending on the role, employees may work from home, onsite or both. This position may work from home and travel to the Mercer, McLean, and Ward County areas.

Minimum Qualifications
You must have your licensure as a Licensed Baccalaureate Social Worker (LBSW) by the ND Board of Social Work Examiners or you must have your licensure as a Baccalaureate of Nursing and licensure for RN (BSN/RN) by the North Dakota Board of Nursing. Also required is one year of professional human services work experience as a social worker human relations counselor, developmental disabilities case manager, mental illness case manager, vocational rehabilitation counselor, activity therapist, addiction counselor, registered nurse, employment counselor, or a similar professional level position in the public or private sector, or a Master's Degree in social worker and licensure as a Licensed Social Worker by the ND Board of Social Work Examiners.
