Home based case manager/visit supervisor

RainTree Consulting is seeking professionals at the Bachelors level who are dedicated to helping change the lives of families in their communities.

Candidates may apply if have some college plus 2 years experience working in direct care services.

some job duties include:

*Complete assessments on referrals - identifying needs

*Provide case management in the home, office or other natural setting. This may include linking to community resources, assisting with employment and housing needs; supervising visits; foster parent support; teaching parenting skills, etc

*Document all contacts and complete progress notes for services

*Prepare reports and communicates with collateral contacts and placing agencies

*Develop a treatment plan

*Attending family team meetings

*Participate in supervision/staffing with supervisor
