Home Care Nurse

We are looking for a Home Care Nurse who provides direct patient care in the home environment, assisting patients in managing their health conditions, promoting recovery, and enhancing their quality of life. Nurses will follow individualized care plans, educate patients and families, and ensure that care is delivered in accordance with medical protocols and standards.

Responsibilities:

• Conduct thorough assessments of patients' physical and mental health.
• Monitor vital signs, symptoms, and patient progress.
• Administer medications and treatments as prescribed by physicians.
• Develop and update personalized care plans based on patient needs.
• Coordinate with physicians, therapists, and other healthcare professionals involved in the patient’s care.
• Educate patients and their families on how to manage their conditions, medications, and therapies.
• Provide guidance on maintaining hygiene, mobility, and overall well-being.
• Maintain accurate and detailed records of patient assessments, interventions, and progress.
• Report any significant changes in patient condition to the healthcare provider or supervisor.

Qualifications:

• Bachelor’s degree in Nursing (BSN) or equivalent qualification.
• Valid QCHP.
• Minimum of 02-03 years of experience in nursing, with at least 1 year in home care or a similar setting
• Strong clinical skills with the ability to assess, plan, and implement nursing care.
• Excellent communication and interpersonal skills to work with patients and families.
• Knowledge of infection control, patient safety protocols, and emergency response procedures.
