Duties

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts or wheelchairs.
  • Monitor patient health by observing vital signs like blood pressure, pulse, temperature, and respiration; document findings accurately in electronic health record (EHR) or electronic medical record (EMR) systems.
  • Administer medications following prescribed schedules and medical terminology guidelines while maintaining strict compliance with HIPAA regulations for patient privacy.
  • Support clients with specialized needs such as Alzheimer’s care, dementia care, memory care, hospice care, or working with individuals with developmental disabilities or disabilities.
  • Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients.
  • Utilize medical documentation skills to record patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans.
  • Operate medical equipment such as catheters or Hoyer lifts safely and competently; assist with transfers and mobility using proper techniques to prevent injury.
  • Provide emotional support and companionship to foster a positive environment that promotes mental well-being and social engagement.
  • Maintain compliance with all healthcare regulations including HIPAA standards; participate in ongoing training on CPR certification and other safety protocols.

Skills

  • Strong patient observation skills to detect changes in health status promptly and accurately.
  • Extensive experience working in assisted living facilities, nursing homes, or home health environments.
  • Proficiency with EMR/EHR systems for documentation of patient care activities and medical records management.
  • Knowledge of assisting with ADLs and providing comprehensive senior care tailored to individual needs.
  • Familiarity with medical terminology relevant to home health care procedures.
  • Ability to perform CPR and handle emergency situations confidently; experience with hospice or palliative care is a plus.
  • Competence in catheter care, medication administration, vital signs measurement, and dementia or Alzheimer’s-specific strategies.
  • Experience working with individuals with disabilities or developmental challenges in community or home settings.
  • Capable of performing light housekeeping tasks such as cleaning, laundry, cooking nutritious meals, and maintaining a hygienic environment.
  • Knowledge of long-term care practices and home & community-based services that promote independence and quality of life for clients.
