Position Summary
The Home Health Aide provides personal care and support to patients in their homes or care settings. Responsibilities include assisting with hygiene, daily living tasks, safety monitoring, and household chores as defined in individualized care plans. This role promotes comfort, hygiene, safety, and emotional well-being while maintaining patient confidentiality and adhering to all organizational and regulatory standards.
PRN as Needed
County Service Area: Duval, St. John's, Clay, Nassau
$19 an hour with competitive mileage reimbursement
Essential Functions
- Assist patients with activities of daily living and personal care as outlined in care plan
- Provide light housekeeping and laundry tasks
- Offer emotional support to patients and families
- Observe and report changes in physical or emotional status to the RN
- Identify and report safety hazards
- Maintain accurate records in electronic medical record (EMR) systems
- Attend staff meetings and complete required training
- Communicate patient-related information effectively to care team
- Follow infection control, HIPAA, OSHA, and company confidentiality policies
- Respond appropriately in emergency situations
- Perform other duties as assigned
Knowledge and Critical Skills
- Knowledge of home health aide practices, medical equipment use and safety precautions
- Ability to communicate effectively and compassionately
- Ability to manage challenging behaviors and react appropriately in emergencies
- Strong confidentiality and ethical standards
- Basic computer literacy
Experience
- 6 months’ experience providing in-home patient care preferred
- 1+ year experience working with frail or elderly individuals preferred
License / Certification
- Current Certified Nursing Assistant (CNA) license in the state of Florida.
- Current CPR/BLS certification.
- Valid Florida driver’s license with acceptable driving record.
- Reliable transportation and current auto insurance
Education
- High school diploma or GED equivalent preferred.
Other Requirements
- TB test and immunization records (or declinations) for Hepatitis B and flu
- Level II background screening and drug test
- Availability of one or more active phone numbers for contact
