Home Health Nurse (LVN)

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our growing home health team on a per-visit, part-time basis. This role is ideal for a nurse who is experienced in providing high-quality care in a home setting and is comfortable working independently while delivering excellent patient outcomes.

Responsibilities:

• Provide skilled nursing care to patients in their homes
• Perform and manage wound care treatments in accordance with physician orders
• Conduct patient assessments and monitor changes in condition
• Administer medications and treatments as prescribed
• Educate patients and caregivers on disease management and care plans
• Maintain accurate and timely documentation

Qualifications:

• Current and active LVN license in CA
• Home health experience preferred
• Strong experience in wound care and patient care
• Ability to work independently and manage time effectively
• Excellent communication and interpersonal skills
• Reliable transportation and valid driver’s license
