Home health offers the opportunity to provide one-on-one care where patients are often most comfortable—their home. Instead of treating multiple patients at once, you'll have the ability to focus on individualized care plans, build meaningful relationships, and help patients achieve goals that directly impact their daily lives.

As a Home Health Physical Therapist, you will evaluate patients in their homes and develop treatment plans designed to improve mobility, strength, balance, safety, and overall function.
Your responsibilities will include:
Performing physical therapy evaluations and reassessments
Identifying mobility limitations and functional deficits
Creating individualized treatment plans based on patient needs and goals
Implementing interventions focused on strength, balance, gait training, and fall prevention
Teaching patients and caregivers exercises and safety techniques
Monitoring progress and modifying treatment plans as appropriate
Coordinating care with physicians, nurses, caregivers, and other healthcare professionals
Completing documentation in accordance with Medicare home health requirements
Successful candidates typically possess:
Current Hawaii Physical Therapist license
Minimum one year of clinical experience
Home health experience preferred but not required
Strong communication and patient education skills
Ability to work independently and manage a field-based schedule
Reliable transportation and valid driver's license
Commitment to providing compassionate, patient-centered care
