POSITION DESCRIPTION

Work includes providing client driven empathetic case management to pregnant families or families with children under 18 months. Case management services are provided to caseload primarily via home visiting. Care will be provided via structured care plans determined by client risk assessment. Work will also include case review, case consultation and visits with families who are case managed by other professional team members (nurses or social workers). Strong organizational skills and self-motivation needed to maximize opportunity for pay.

POSITION REQUIRED QUALIFICATIONS

• Successful candidates must be able to provide at least 10-15 home visiting case management visits
• Must have reliable smart phone, tablet and/or computer for documentation in electronic medical record
• Must have driver's license and vehicle in good working order
• Must have strong decision making and problem solving skills
• Must have excellent communication skills both written and verbal
• Familiarity with resources and social service agencies in county of service is preferred.
• Must be computer literate in Google Docs, Google Calendar, E-mail and the Internet, Electronic health record experience preferred
• Must be reliable and professional in all aspects of work
• Must have a flexible schedule
• Must be culturally competent and work well in a team environment

POSITION REQUIRED EDUCATION AND EXPERIENCE:

• Candidate must have and maintain a current Michigan registered nurse license (RN)
• At least one year of experience providing services to families or case management experience is strongly preferred. Extra training is available and will be required in lieu of experience requirement. Incentive payment for required independent training. Payment received upon completion of all required training and completion of 10 independent visits within 30 days of hire.
• MIHP experience is strongly preferred

FULL-TIME/PART-TIME

Part-time to Full-time, flexible, average 10-15 visits per week minimum, independent contractor

SCHEDULE

Flexible, day time and evening availability, set your own hours

LOCATION

Home Visits in concentrated area of Wayne County and administrative work from your home office or a confidential location, must have reliable vehicle and willingness to drive to client home for home visits. Must have adequate internet service to provide telehealth/virtual/video conference visits and documentation in EMR system. Gas and mobile monthly stipend available in addition to compensation.

Pay: $35.00 - $50.00 per hour
