Utilizes the Nursing Process in MCH-CHPS, through assessment of the health of clients. Documents care/services when provided per Agency Policies/Procedures.
Implements care plans to include appropriate interventions, education, and counseling based on identified needs; and initiating/follow up on referrals when indicated. ldentifies patient needs through Maternal Child Health Services.
Performs MCH-CHPS services, including Postpartum Newborn Home Visits and Specialty Tracking such as Congenital Syphilis, Elevated Blood Lead Levels Case Management, and Newborn Screening Case Management.
Coordinates with MCH-CHPS Program Manager on monthly MCH-CHPS services through reports and completes peer audits.
Promotes and supports the development of programs, policies, and services that provide interventions that improve the health status of maternal and child populations.
Emergency Operations
Willing and available to assist during limes of public health emergency and outbreak response efforts as indicated by DPH.
Additional Duties as Assigned.
State Minimum Requirements: Graduation from an accredited school of nursing.

DRIVING RECORD: If this position requires the applicant to possess a valid driver's license to operate a state vehicle or personal vehicle, any applicant being considered in the final stages of selection for the position will be required to provide a certified copy of a 10-year driving record.
