Homeless Case Manager

**Job Title:** Case Manager - Housing Program Operations

BARE Truth is committed to supporting individuals and families experiencing homelessness. Our Interim Housing Program provides crucial temporary shelter and facilitates transitions to stable, permanent housing.

**Job Summary:**

The Case Manager will provide comprehensive case management services to individuals facing homelessness. This role involves conducting thorough assessments, developing individualized housing stability plans, and providing ongoing support to ensure participants' successful transition to permanent housing.

**Key Responsibilities:**

- Conduct in-depth assessments of participants' health, emotional, social, and housing needs.

- Develop and implement personalized housing stability plans for each participant.

- Provide continuous case management, including problem-solving, advocacy, and resource linkage.

- Collaborate with housing navigators, healthcare providers, and other service agencies to provide holistic support.

- Monitor and track participants' progress, making necessary adjustments to their plans.

- Maintain accurate and timely documentation of all activities and participant interactions.

- Participate in team meetings to ensure coordinated and effective service delivery.

- Provide crisis intervention and conflict resolution when necessary.

- Serve as a liaison with landlords, property managers, and other housing stakeholders.

**Qualifications:**

- Bachelor’s degree in Social Work, Healthcare Administration, Public Health, or a related field preferred.

- Experience in case management with vulnerable populations, especially those experiencing homelessness.

- **Proficiency in both English and Spanish is required.**

- Strong interpersonal, communication, and organizational skills.

- Commitment to working compassionately with diverse populations.

- Must be able to pass a criminal background check and drug test.

**Working Conditions:**

- The role involves a combination of office and fieldwork, including visits to interim housing locations.

- Some flexibility in schedule, including the potential for occasional evening or weekend hours.

**Job Types:** Full-time, Contract

**Pay:** $25.00 - $30.00 per hour

**Expected hours:** 40 per week

**Schedule:**

- 8-hour shift

- Evenings as needed

- Monday to Friday

- Weekends as needed

**Ability to Relocate:**

Los Angeles, CA 90001: Relocate before starting work (Required)
