Hospice Community Liaison

Position Summary

The Hospice Community Liaison is responsible for developing and maintaining referral relationships, increasing patient admissions, and representing KH Prime Hospice Care across the Sacramento market.

You will work closely with hospitals, physicians, and care facilities to educate partners on hospice services and ensure seamless patient transitions.

Key Responsibilities

• Develop and maintain strong referral relationships with:
• Hospitals
• Physicians and specialty clinics
• Skilled Nursing Facilities (SNFs)
• Assisted Living Facilities (ALFs)
• Board & Care and RCFE facilities
• Generate consistent patient referrals and meet or exceed monthly census growth targets
• Serve as the primary point of contact for referral sources and community partners
• Educate healthcare providers, patients, and families on hospice benefits and eligibility
• Coordinate with clinical teams to ensure smooth admissions and transitions of care
• Track referral activity, maintain CRM updates, and report performance metrics
• Identify market opportunities and competitor activity
• Represent the organization at networking events, facility visits, and community outreach initiatives

Qualifications & Requirements

• LVN, RN, or MSW preferred but not required (strong marketers encouraged to apply)
• Minimum 1–2 years of hospice marketing experience required!
• Existing relationships with local referral sources is required!
• Strong understanding of hospice eligibility and Medicare guidelines preferred
• Excellent communication, negotiation, and interpersonal skills
• Self-motivated, goal-oriented, and able to work independently
• Reliable transportation and willingness to travel within the service area

Compensation & Benefits

• Base Salary: $100,000 – $120,000+ (based on experience)
