Description

American Traveler is seeking an experienced RN Case Manager for a home health hospice position requiring Epic EMR experience and a valid driver's license for community-based patient visits.

Details

  • Home health hospice setting serving patients in private homes, assisted living facilities (ALFs), SNFs, group homes, independent living communities, and two hospital locations for general inpatient (GIP) care
  • Patient population consists of end-stage/hospice patients
  • Caseload of 14–16 patients per RN Case Manager; team includes 7 RNCMs and 6 CNAs
  • Floating is required within the community to patient homes
  • 1st shift, Monday through Friday, 8:00 AM to approximately 4:30 PM (40 hrs/week)
  • No weekend rotation, no on-call, and no holiday requirements
  • EMR: Epic and Rover; Epic experience required

Requirements

  • Active WI or compact RN license currently in hand
  • BLS certification required
  • Valid driver's license and current auto insurance required
  • Minimum 2 years of RN experience, with at least 12 months of core experience in home health or hospice specialty
  • Most recent experience must be within the last year
  • Epic EMR experience required
  • 1 professional reference dated within the past year from a Supervisor, Manager, Director, or Charge RN is required for consideration
  • Last 4 digits of SSN and full date of birth required for consideration

Additional Information

  • Provides RN case management to hospice patients across a variety of community and facility settings, including homes, ALFs, SNFs, and hospitals
  • Uses home health bag and trunk supplies, IV pumps, medication dispensing systems, and chair/bed alarms as part of daily care
  • One week of unit orientation provided
  • Scrub colors: blue or black
  • Free parking available at the Burlington and Walworth office locations
  • Team-oriented unit culture with strong team support
  • First-time travelers are not preferred; prior hospice or home health travel experience is recommended
