Hospice Social Worker

Job Summary
The MSW will provide direct psychosocial support and counseling to hospice patients and families, helping them navigate the emotional, social, and practical challenges of terminal illness. You will serve as a key member of our interdisciplinary care team, working closely with nurses, physicians, chaplains, and aides to ensure holistic care.

Key Responsibilities

  • Conduct initial psychosocial assessments of hospice patients and families.
  • Develop individualized care plans addressing emotional, social, and resource needs.
  • Provide ongoing counseling, grief support and crisis intervention.
  • Assist with end-of-life planning, advance directives, and community referrals.
  • Support patients and families through the bereavement process as needed.
  • Collaborate with interdisciplinary team to ensure coordinated, compassionate care.
  • Maintain accurate and timely documentation in accordance with agency and regulatory requirements.
  • Participate in case reviews and multidisciplinary team meetings to discuss cases.

Qualifications

  • Master’s degree in Social Work (MSW) from an accredited institution.
  • Valid social work license (LCSW or equivalent) required.
  • Minimum of one year experience in healthcare or hospice setting preferred.
  • Knowledge of grief counseling, family dynamics, and hospice philosophy preferred.
  • Proficiency in crisis management and intervention strategies.
  • Excellent communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
  • Valid drivers license, reliable transportation, and willingness to travel to patient homes.
