Position Overview:

The Housing Case Manager will be responsible for helping participants with severe and persistent mental illness to secure and maintain affordable housing. The Housing Case Manager will participate in housing development activities to ensure that subsidy amount is accurate for each participant. They will also provide housing advocacy and other supportive services to assist clients to meet tenancy obligations, mediate landlord-tenant issues, home visits and services to assist clients in maintaining housing. The Housing Case Manager will work closely with the mental health providers to assure coordination of care and overall client success.

Essential Duties & Responsibilities

• Outreach to landlords and consumers to market the program and to develop relationships to maintain a housing stock that is appropriate for our participants.
• Assess participants by identifying and addressing housing barriers.
• Assist participants with searching and locating appropriate housing, completing rental applications, scheduling interviews and helping to coordinate the consumer’s benefits to ensure that the subsidy amount is accurate.
• Provide advocacy and other supportive services to help participants fulfilling lease requirements such as budgeting, house rules and maintenance requests.
• Aid in the mediation of landlord-tenant issues, provide home visits and help consumers apply for and transition to other long-term housing utilizing Section 8 vouchers and Continuum of Care vouchers.
• Prepares reports and coordinate applications to subsidy program.
• Enter documentation of all work completed into electronic medical record within 2 business days.

Physical Requirements :

• The ability to meet the following physical requirements with or without reasonable accommodation:
• This position requires spending a majority of the workday standing and walking, with occasional sitting.
• Use of hands, wrists and fingers and lifting or moving up to 25lbs.
• Movements such as stooping, crouching, bending, kneeling, climbing and reaching are occasionally required.
• Noise and/or level of distractions in the work environment is moderate.
• Specific vision abilities required by this position include close vision, distance vision and peripheral vision.
• Talk, hear and communicate with clients and put client’s symptoms and functioning in the context of their mental illness.
• Ability to operate a motorized vehicle.

Minimum Requirements

Education

• Bachelor’s Degree in social or human services preferred

Experience

• 1 year experience working in the field.

Licenses

• Must have a current driver’s license and the ability to meet Sequoia’s driving requirements.

Job Type: Full-time

Pay: $48,750.00 - $52,650.00 per year
