Position Overview:
The Housing Case Manager will be responsible for helping participants with severe and persistent mental illness to secure and maintain affordable housing. The Housing Case Manager will participate in housing development activities to ensure that subsidy amount is accurate for each participant. They will also provide housing advocacy and other supportive services to assist clients to meet tenancy obligations, mediate landlord-tenant issues, home visits and services to assist clients in maintaining housing. The Housing Case Manager will work closely with the mental health providers to assure coordination of care and overall client success.
