Job Overview
We are seeking a compassionate and dedicated In-Home Care Provider to deliver exceptional care and support to individuals in their own homes. This vital role involves assisting clients with daily activities, ensuring their safety, and promoting independence. As an In-Home Care Provider, you will play a crucial part in fostering a safe, comfortable, and nurturing environment for clients with diverse needs, including seniors, individuals with disabilities, or those recovering from illness or injury.
Duties
- Provide personalized assistance with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts.
- Monitor and observe patients’ health status by recording vital signs and noting changes in condition or behavior.
- Administer medications accurately according to medical instructions while maintaining detailed medical documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Assist with medical procedures including catheter care, wound management, and hospice or dementia-related care such as Alzheimer's and memory care.
- Support clients with meal preparation, feeding, light housekeeping tasks including cleaning and laundry to maintain a safe and sanitary environment.
- Document all care activities thoroughly while adhering to HIPAA regulations to ensure patient confidentiality.
- Use medical terminology proficiently when recording patient data and communicating with healthcare teams.
- Employ patient observation skills to identify potential health concerns early and escalate issues appropriately.
- Operate home health equipment safely, including Hoyer lifts and other assistive devices.
- Work compassionately with individuals who have developmental disabilities or require specialized memory care services.
- Maintain compliance with all safety protocols during patient care activities and emergency responses.
Qualifications
- Proven experience in home & community care settings or long-term care environments such as nursing homes or assisted living facilities.
- Strong knowledge of patient care procedures including medication administration, vital signs monitoring, catheter care, and dementia or Alzheimer’s care.
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping.
- Valid CPR certification; additional certifications such as medical terminology training or hospice care are a plus.
- Ability to work independently while following detailed care plans and instructions from healthcare professionals.
- Excellent communication skills to effectively collaborate with clients, families, and healthcare teams while maintaining confidentiality under HIPAA guidelines.
- Physical ability to assist with mobility support using lifts or other equipment; stamina for long shifts involving standing, walking, or lifting patients as needed.

Pay: $13.00 - $16.00 per hour
