Summary:

The Intake Coordinator/Case Manager position is responsible for the coordination and management client direct care services. The Intake Coordinator/ Case Manager will provide programmatic information clients and external customers. The Intake Coordinator/Case Manager will be responsible for developing and reviewing appropriate and timely documentation for all services provided. The Intake Coordinator/Case Manager will be in compliance with all guidelines imposed by the Department of Corrections.
Essential Duties and Responsibilities:

Provide all Intake duties including: Initial Intake; Assessments; Referrals; Evaluations; PREA documentation; Bed assignments; Census (population) sheets; Client handbook; orientation to program structure and Consents

Complete Entitlement documentation for procurement of Medicaid.

Provide and document all case management duties including: Re-Entry Plans, Re-Entry Plan Reviews, Progress Notes, Fee Collection and Documentation; Client schedules; Employment verifications; individual counseling and evaluations.

Provide client direct care services including: facility head counts and tours; monitoring of self administration of medication; client accountability checks and facility/grounds safety.

Completion of mandatory trainings

Knowledge and understanding of all Department of Corrections contractual agreements in accordance with the Residential Provider Manual
Required Knowledge, Skills, Abilities and Other Characteristics:
Knowledge of:

Electronic Healthcare Record systems. ·

General office software, particularly Microsoft Office Suite
Ability to:

Interact effectively with representative of funding sources and referral agencies.

Analyze and solve problems effectively and efficiently.

Communicate effectively both verbally and in writing.

Manage priorities to meet deadlines and demonstrate multi-tasking skills.

Education and Experience:

Bachelors Degree with a concentration in the area of Counseling or Criminal Justice

Experience within the criminal justice environment
Must be:

ethical, self-motivated, dependable/reliable and detailed oriented. ·

Must be able to interface easily with clients, staff, and other management. ·

Must be able to meet routine office physical demands including climbing stairs, carrying up to 20 pounds objects and possessing the dexterity and vision to operate office computers and equipment. · *

Must possess a Valid driver's license
