Job Purpose
-
Responsible to deliver a professional, safe and quality patient care through the nursing process of assessment, planning, implementation and evaluation.

Accountabilities
Key Responsibilities: Service
-
Deliver safe and quality nursing care in conformance with the Sir HN RFH policies and processes.
-
Ensure patient safety at all times according to hospital policies
-
Ensure that patient privacy and confidentiality is duly protected at all times
-
Treat patients and their family with courtesy, respect, and caring behavior
-
Take immediate action to resolve any issues, complaints or distress expressed in any way by the patient or their family with in her limits and escalate to higher level
-
Receive the patient, check the identification, 0rient to the admitted unit and make the patient comfortable
-
Conduct patients initial nursing assessment within 30 mints of arrival in ward and complete the assessment within 2 hours. An ongoing nursing assessment to be done in the beginning of each shift. Reassessment to be done for long stay patients after every 30 days
-
Demonstrate clinical skills and knowledge in the delivery of patient care on various nursing procedures
-
Ensure smooth functioning of the ICU
-
Complete the pain assessment and reassessment and intervene as required
-
Identify the vulnerable patients, plan and provide the care accordingly
-
Assess the patient for risk of developing pressure ulcer and implement pressure ulcer preventive measures
-
Administer medication to the patient as per the doctors prescription and document the same in EMR
-
Accompany the Consultants in the clinical rounds and follow the orders correctly. Assist the doctors in bedside procedures
-
Provide Basic Life Support in all emergency situations
-
Demonstrate critical thinking skills which is essential to make accurate decisions in patient care
-
Ensure safe care and handling of patients on invasive lines and tubes
-
All critical alerts to be read, verified and documented with clinical intervention
-
Coordinate admission, transfer and discharge process ensuring adherence to defined timelines
-
Ensure provision of end of life care to patients those are in end stage
-
Maintain accurate, comprehensive and timely medical records
-
Activate CODE Blue if patient is unresponsive
-
Identify early warning signs of the patient and activate MET calls as per the defined role
-
Participate actively in case of any internal / external disaster situations
-
Conduct hourly/frequent round as per the needs of the all patients
-
Maintain Patient hygiene. Provide sponge bath to post-operative patients, critically ill and bed bound patients
-
Accompany patients with High acuity, patients with tubes and lines and bed bound patients for transfer
-
Educate and involve the patient and family regarding the plan of nursing care during the hospital stay and at the time of discharge
-
Ensure all clinical consents are uploaded in patient EMR
-
Ensure safe handling and handing over of all diagnostic reports at the time of discharge and document the same in discharge checklist
-
Ensure MLC is initiated for all MLC cases and the process of discharge intimation is given to PE. All the documentation is done and followed as per hospital policy.

Check MRD file updated and complete and handover the same to MRD within 24 hrs. of discharge
-
Provide death care to the deceased patients respectfully and handover to relatives/mortuary as required and document the same
-
Any other responsibility as assigned by the ICU manager

Actively participate in infection control and prevention activities, campaigns, awareness and sessions

Effective optimal utilization of available resources ( drugs and consumable )

Ensure proper utilization of ward stock items and maintain its appropriate charging or stock consumption record

Ensure safe handling of CSSD and biomedical equipments and prevent breakages

Attend all in service Nursing/hospital trainings
-
Assist in orientation of new nursing staff and supervise them for safe patient care

Supervise PCAs / PTAs and ensure patient care

Ensure effective patient care delivery and achievement of desired patient outcomes in

collaboration with other professional disciplines

Engage in training and educational program for freshers and new joinees to impart the protocols and hospital policy

Ensure effective communication with the health care team members by ensuring:

Read back and verify Verbal /Telephonic orders/ critical alert reporting. No verbal order to be followed

SC Degree in Nursing from a recognized nursing institute.

RELEVANT EXPERIENCE:
Minimum 3 to 4 years of clinical experience in the relevant area
Desirable:
For super specialist/ specialist areas minimum 2 years experience in respective work area
