Overview
We are seeking a dedicated Licensed Alcohol & Drug Counselor (LADC / ADC-T or equivalent) to join our team in Eagan, Minnesota. This role offers an exciting opportunity to make a meaningful impact by providing evidence-based addiction counseling and behavioral health services within a supportive outpatient environment. Sage Prairie offers ASAM levels 1.0, 2.1 and 2.5. The counselor will work closely with clients to facilitate recovery, utilizing clinical expertise in addiction medicine, mental health counseling, and crisis management.

Responsibilities

  • Conduct thorough patient assessments, including diagnostic evaluations and intake procedures, to determine individual treatment needs.
  • Develop personalized treatment plans incorporating psychotherapy, cognitive behavioral therapy, motivational interviewing, and group therapy techniques.
  • Provide individual and group counseling sessions focused on addiction recovery, mental health support, and relapse prevention.
  • Implement crisis intervention strategies to address urgent behavioral health issues and ensure client safety.
  • Document all patient interactions accurately within HIPAA-compliant medical records systems, utilizing appropriate medical terminology and ICD coding standards (ICD-9/10).
  • Collaborate with multidisciplinary teams to coordinate case management efforts and facilitate seamless patient care across home, community, hospital, and outpatient settings.
  • Engage in ongoing professional development related to addiction counseling, behavioral health advancements, and research updates in the field of mental health.

Experience
Candidates should possess a valid license as an Alcohol & Drug Counselor (LADC / ADC-T or equivalent) with demonstrated experience in clinical counseling within behavioral health or addiction treatment settings. Prior experience in crisis management, hospital environments, or home/community care is highly desirable. Familiarity with medical documentation standards—including ICD coding—and proficiency in applying evidence-based practices such as motivational interviewing, behavioral therapy, applied behavior analysis, and trauma-informed care are essential. A strong foundation in case management, patient assessment techniques, and familiarity with CPT coding for billing purposes will enhance effectiveness in this role.
