About the Role

Val Verde Regional Medical Center (VVRMC) is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join our Case Management team. In this role, you will provide critical psychosocial support to patients and families navigating complex medical, emotional, and end-of-life care needs.

As a community-owned, nonprofit hospital, VVRMC is deeply committed to delivering patient-centered care. This position plays a vital role in supporting patients across the continuum of care—including inpatient, discharge planning, and hospice—while also providing bereavement services to families.

What You’ll Do

  • Assess patients’ and families’ psychosocial needs, including emotional, social, cultural, and financial factors
  • Develop and implement individualized care plans in collaboration with the interdisciplinary care team
  • Provide counseling and emotional support for patients and families coping with illness, trauma, or end-of-life care
  • Coordinate discharge planning, including follow-up care and community resource referrals
  • Conduct social histories and evaluate patient/family dynamics and support systems
  • Participate in interdisciplinary team meetings, case conferences, and daily huddles
  • Provide bereavement counseling and support for families for up to 13 months following hospice patient death
  • Develop and manage bereavement plans of care, including support groups, memorial services, and outreach
  • Maintain accurate, timely documentation within the electronic health record
  • Educate healthcare team members on social service and patient advocacy needs
  • Assist patients and families in navigating insurance coverage and financial resources
  • Coordinate with community agencies and external providers to ensure continuity of care
  • Support guardianship processes for patients without decision-making support
  • Participate in quality improvement, compliance, and performance initiatives
  • Provide occasional emergency department coverage and participate in on-call rotation

What We’re Looking For

  • Master’s Degree in Social Work (MSW) from an accredited program (preferred)
  • OR
  • Bachelor’s degree in social work, psychology, sociology, or related field (under supervision of MSW)
  • Current Texas Social Worker license (LMSW) in good standing
  • Minimum of 2–4 years of experience in healthcare, hospice, home care, or case management
  • Strong assessment, counseling, and care coordination skills
  • Ability to work independently and make sound clinical judgments
  • Excellent communication, documentation, and interpersonal skills
  • Ability to thrive in a multidisciplinary, fast-paced healthcare environment
