**This is a full-time onsite position at our PACE center in Los Angeles, CA 90057**
As a Licensed Physical Therapist Assistant (LPTA) in our PACE program, you’ll play a vital role in helping older adults maintain their independence, mobility, and overall well-being. Working under the guidance of a Physical Therapist and as a valued member of our Interdisciplinary Team (IDT), you’ll bring therapeutic care to life—delivering compassionate, hands-on support tailored to each participant’s unique goals.

Responsibilities:

  • Hands-On Healing: Deliver one-on-one, personalized therapy interventions following care plans created by the Physical Therapist.
  • Mobility Mentorship: Guide participants through therapeutic exercises, balance training, gait support, and physical modalities that improve daily function and quality of life.
  • Attentive Observation: Monitor participants during sessions, track progress, and share valuable insights with the Physical Therapist to support continuous care planning.
  • Respectful, Person-Centered Care: Create a safe, comforting, and empowering space during each session—adapting your techniques to each individual’s needs.
  • Supportive Education: Teach participants and caregivers how to safely use assistive devices, practice mobility techniques, and follow home exercise programs with confidence.
  • Collaborative Spirit: Work closely with the Physical Therapist and other members of the Interdisciplinary Team to ensure coordinated, holistic care for every participant.
  • Continuity of Care: Support transitions between home, hospital, or skilled nursing settings by helping ensure therapy services stay on track and goals are maintained.
  • Welcoming Environment: Keep therapy spaces clean, organized, and inviting—creating a place where participants feel safe and supported.
  • Reliable Documentation: Accurately record interventions and participant responses in the Electronic Medical Record (EMR) in accordance with PACE, state, and federal requirements.
  • Team Engagement: Actively participate in daily huddles, team meetings, and training sessions to stay informed, aligned, and inspired.
  • Flexible Support: Step in to assist with other participant care or team needs as they arise—bringing a helpful attitude and a willingness to contribute wherever you're most needed.

Qualifications and experience:

  • Qualified & Ready to Care: Graduate of an accredited PTA program with an active California license required.
  • Prepared for the Unexpected: CPR certification (or willingness to obtain within 30 days of hire).
  • Experienced in Geriatric Care: At least 2 years of PTA experience, preferably with older adults or medically complex participants.
  • Rooted in Purpose: A genuine passion for supporting frail older adults through compassionate, person-centered care.
  • Strong Communicator: Ability to engage participants, caregivers, and teammates with empathy, clarity, and positivity.
  • Adaptable Clinician: Comfortable adjusting interventions based on participant needs, goals, and physical capacity.
  • Confident in Hands-On Support: Skilled in mobility assistance, transfers, and therapeutic interventions.
  • Detail-Oriented Documenter: Organized and timely when it comes to recording care in the EMR and managing schedules.
  • Tech-Savvy: Proficient with electronic medical records and basic computer applications.
  • Reliable Teammate: Punctual, dependable, and always ready to pitch in with a kind and can-do attitude.
  • Onsite Presence – This is a 100% onsite position supporting our vibrant South Los Angeles market. Your in-person presence is key to building strong team connections and delivering high-quality, coordinated care.
  • Consistent Schedule – The regular work schedule is Monday through Friday, from 8:00 AM to 5:00 PM, supporting a stable routine for both staff and participants.

Tuberculosis Screening Policy
