Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusion and injection administration.
- Monitor patients’ conditions closely, utilizing medical terminology and physiology knowledge to identify changes requiring intervention.
- Assist with medical procedures such as catheterization, suturing, tube feeding, and sterile processing techniques.
- Document all patient interactions thoroughly using EMR (Electronic Medical Record) systems like Cerner or eClinicalWorks, ensuring compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care strategies to optimize patient outcomes.
- Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care and behavioral health support.
- Support emergency situations with airway management, ventilator operation, and acute pain management in high-pressure environments such as ICU or Level I Trauma Centers.
Skills
- Proven experience working with diverse patient populations in settings such as hospital medicine, critical care (ICU), emergency room (ER), or long-term care facilities.
- Strong knowledge of ICD-9/10 coding systems for accurate medical documentation and billing processes.
- Skilled in triage assessment and urgent care procedures to prioritize patient needs efficiently.
- Experience with medical imaging procedures like sonography or laboratory specimen collection/processing is a plus.
- Familiarity with EMR/EHR systems including Athenahealth or NCQA standards for seamless documentation workflows.
- Expertise in infection control practices, aseptic techniques, and sterile processing to maintain patient safety standards.
- Ability to work with individuals with disabilities or developmental delays in community or home care settings.
- Competence in performing venipuncture (venipuncture), phlebotomy, catheter care, and basic life support (BLS).
- Knowledge of anatomy and physiology essential for accurate physical examinations and diagnostic evaluation.
- Excellent communication skills for patient education on health coaching topics such as nutrition or medication management.
