We are looking for a Licensed Social Worker to join our team. The ideal candidate will have experience providing counseling services to long term care residents. The successful candidate will be able to effectively collaborate with other professionals in the field, possess strong communication skills, and have a passion for helping others.

Responsibilities:

• Provide psychotherapy to residents and families.
• Conduct assessments and develop care plans within an Interdisciplinary Team.
• Provide crisis intervention and case management services.
• Maintain accurate and up-to-date records of client progress.
• Collaborate with other professionals to coordinate services for clients.
• Assist families with nursing facility Medicaid applications.
• Coordinate counseling, dental, vision, and podiatry services.
