LPN (Contract to Perm) – NY 13165

• Contract Length: 13 Weeks (05/12/2026 – 08/15/2026)

• Schedule: Full-Time, 5x8-hour night shifts

• Shift Time: Nights (e.g., 11:00 PM – 7:30 AM)

• Hours: 40 hrs/week

• Pay: $1,258 – $1,420/week

• Requirements: Every Other Weekend (EOW) + Holidays required

Position Summary & Responsibilities

• Provide skilled nursing care to adult/geriatric residents under RN supervision

• Assess, monitor, and report physical/psychosocial status changes

• Administer medications/treatments; assist with ADLs per care plan

• Contribute to care plans; collaborate with interdisciplinary team

• Maintain documentation, safety standards, and infection control

Requirements

• Active NY LPN Licensure

• BLS Certification (AHA or RQI required)

• Minimum 1 year LPN experience (LTC preferred)

• Ability to care for adult and geriatric populations

• Graduate of accredited LPN program

• Willingness to work in environment with potential exposure to blood/body fluids
