We support youth experiencing housing instability, family conflict, system involvement, exploitation, and other challenges, while also providing prevention and engagement opportunities that help build belonging and possibility before a crisis occurs.

Job Summary

The Maternity Group Home Case Manager walks alongside pregnant and parenting youth experiencing homelessness or housing instability as they build safety, stability, and confidence in their futures. In this role, you partner with youth to identify their strengths, explore their goals, and create individualized plans that support housing stability, emotional well-being, healthy relationships, independent living skills, and long‑term self‑sufficiency.

Case Managers honor youth as the experts in their own lives and work to build trusting, respectful, and reliable relationships. Through trauma‑informed and strengths‑based approaches, you help youth navigate barriers, connect with community resources, strengthen parenting skills, and access opportunities that support healing and growth. The Case Manager also supports the day-to-day operations of the housing program to ensure a safe, supportive, and empowering living environment for young parents and their children.

Qualifications

A bachelor’s degree in social work, human services, or a related field and two years of experience working with youth are required.

Job Duties

  • Conducting intakes and assessments
  • Working regionally and statewide with CoC members on the Coordinated Entry Process
  • Creating a comprehensive service plan with youth
  • Walk along youth in their navigation of services in the community
  • Lead parenting education individually and in a group setting
  • Create aftercare plans and continue to support youth one year after exit
  • Timely and accurate documentation that supports program quality and accountability

Work Schedule

This is a full-time position. The typical schedule is Monday through Friday 8:30am-5pm with flexibility to meet program needs, including occasional evenings. There are on-call responsibilities.

Compensation & Benefits

Starting salary $45,000 dependent on experience. Benefits discussed during interview.
