Position Summary:

The MDS Coordinator is responsible for managing and coordinating the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations. This role ensures the accurate and timely completion of the Minimum Data Set (MDS) assessments, care area assessments, and individualized care plans to optimize resident outcomes and reimbursement under the Patient Driven Payment Model (PDPM) and state Medicaid case-mix systems.

Key Responsibilities:

  • Coordinate and oversee the completion of the MDS assessments for all residents.
  • Ensure timely and accurate transmission of MDS data.
  • Lead and facilitate the interdisciplinary care plan process, ensuring individualized, person-centered care plans are developed and updated.
  • Monitor PDPM reimbursement drivers and collaborate with nursing, therapy, and other departments to ensure accurate clinical documentation.
  • Track and trend Quality Measures (QMs), Five-Star indicators, and identify opportunities for improvement.
  • Participate in Utilization Review, Triple Check, and interdisciplinary team meetings to ensure compliance and accurate billing.
  • Provide education and training to nursing and interdisciplinary staff on RAI/PDPM guidelines, documentation requirements, and care planning standards.
  • Assist with audits, ADRs, denials, and compliance reviews as assigned.
  • Maintain current knowledge of MDS/RAI regulations, PDPM updates, and state case-mix requirements.

Qualifications:

  • Current Registered Nurse (RN) license.
  • Prior experience as an MDS Coordinator strongly preferred; willing to train the right candidate with strong clinical and documentation background.
  • Solid understanding of RAI guidelines, PDPM, and state Medicaid case-mix methodologies.
  • Strong organizational skills with the ability to manage multiple deadlines.
  • Excellent communication and interpersonal skills to collaborate with all members of the care team.
  • Proficiency with electronic medical records (EMR) and MDS software systems.

Preferred Attributes:

  • RAC-CT (Resident Assessment Coordinator – Certified) certification, or willingness to obtain.
  • Previous experience in long-term care/skilled nursing facility setting.
  • Detail-oriented, analytical thinker with ability to identify trends and drive quality improvement.
