Medical Assistant - Navigator

Overview:
The Medical Assistant Navigator is responsible for replying to patient's messages, primarily through MyChart, to ensure the patient’s health care needs are met and removing barriers to care in a timely fashion. Serves as advocate and medical contact person for patient, family, healthcare team members to assure continuity of care and optimal clinical and resource outcomes.
Responsibilities:
Essential Duties:

  • Provide resources and medical telephonic coaching, information, and educational materials to patients.
  • Provide clinical support to CommUnityCare Provider teams.
  • Act as a liaison between the patient and the health centers.
  • Direct patient flow to ensure patients disposition is accurate as needed.
  • Assist patients with securing appropriate medical care and services within the health system and community and facilitate patient access to medical home providers, staff and resources.
  • Accurately and promptly schedules, reschedules, and cancels physician and non-physician patient appointments.
  • Identify urgent scheduling needs and prioritize them with the appropriate health center or resource.
  • Identify urgent and potentially urgent patient care situations and escalate as appropriate for a positive patient outcome.
  • Participate in orientation of new employees and attend mandatory meetings.
  • Work as a team member which involves interacting with staff, patients, and providers to communicate and coordinate patient care and workflow effectively.
  • Perform other duties as assigned.

Knowledge/Skills/Abilities:

  • High level of skill at building relationships and providing excellent customer service.
  • High level of problem-solving skill to better serve patients and staff.
  • Strong attention to detail and accuracy.
  • Excellent verbal and written communication skills.
  • Demonstrated familiarity with tools, technology, and systems typically found within most health care environments (i.e. personal computer skills, spreadsheets, word processing, patient records systems, EMR systems, etc.).
Qualifications:
MINIMUM EDUCATION:
  • Graduation from high school or equivalent.
  • Medical Assistant Diploma and/or National Medical Assistant Certification verifying graduation from a Medical Assistant Program or
equivalent military healthcare experience.

MINIMUM EXPERIENCE:

  • Six months of experience as a Medical Assistant and/or Care Coordinator in an outpatient medical setting.

REQUIRED CERTIFICATIONS/LICENSURE:

Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red
Cross. Will be required to maintain a current CPR certification during employment.
