Medical Coordinator

The Medical Coordinator is responsible for overseeing key/required medical support for people who receive residential services at EP!C. This person is responsible for ensuring that EP!C is in compliance with Rule 50, Dept. of Human Services Rule 115, Rule 116, Rule 119, Rule 120, Rule 140, and other relevant regulatory guidelines. The medical coordinator is responsible for the direct supervision of the Health Services Direct Support Professionals. The medical coordinator will also ensure that all medical support for individuals align with best practice. The medical coordinator works with the interdisciplinary team to ensure individuals have the best possible health, especially as it relates to assuring that individuals served are scheduled and attend primary care, therapy, specialty care, dental, and/or preventive care as defined by individual needs and regulations. The medical coordinator is a key member of the health services multi-disciplinary team with leading/coordinating on-site clinics for primary care and psychiatric care. The medical coordinator seeks and communicates health status updates to stakeholders at EP!C, including but not limited to individuals who are hospitalized and will need transition planning/follow-up care.

Education/Experience Requirements:
1. High School Diploma or GED
2. Valid Driver’s License or State ID
3. Licensed Practical Nurse, Certified Medical Assistant or Medical Assistant required.
4. Three or more years in a clinical setting preferred with at least one year of supervisory experience preferred.
5. Successfully complete CPR, First Aid, Infection Control, and Crisis Prevention Institute (CPI) training and maintain these certifications.
6. Must complete DSP and Authorized Direct Support Person (ADSP) for medication administration training in accordance with the state regulations and successfully maintain delegated status.

DUTIES / RESPONSIBILITIES

Mission, Vision, and Values

1. Support the agency’s mission, vision, values, and strategic plan philosophy

2. Manage all responsibilities based on the agency's mission, using individual choice, and all appropriate policies, procedures, and regulations

3. Conduct oneself in a way that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.


Assistance to the Director of Nursing:

1.Satisfactorily complete IDHS Medication Administration training.

2. Support the laboratory draw/ordering process for individuals who obtain blood draws through EP!C.

3. Works with medical providers to ensure orders are received for all routine medications and PRN medications.

4. Mentors the team on ensuring all follow-up is completed for ER visits, hospitalizations, urgent care visits, etc.

5. Supports the DON and team in finding resources for all medical needs within each person’s specific insurance.

6. Tracks and reports on departmental data, as needed/requested.

7. Ongoing communication with the pharmacy and/or medical providers to ensure all medication orders occur and there are no gaps in the delivery of medications to people supported.


Staff Supervision

In cooperation with the HR department, conduct interviews, hire, supervise, assign tasks, evaluate performance, and, when necessary, discharge personnel directly and in coordination with relevant department heads.

C. Personnel Responsibilities

1. Interview applicants and look for the skills and experience that meet the needs of the people we support

2. Ensure team trainers and other EP!C professionals orient and train new team members to meet the interests and programmatic goals of persons supported

3. Continuously assess new staff members' skills and address any weaknesses or concerns identified in their job performance

4. Monitor that all staff meet performance expectations and address concerns promptly to ensure peak performance for the person supported

5. Complete all payroll and personnel tasks within the specified timeline. This may include creating schedules, verifying timecards, processing Paid Leave requests, and administering Intermittent FMLA and VESSA

6. Track tardiness and absences, and follow up with any employee issues promptly

7. Ensure that all procedures, protocols, and forms are available and up to date


Department/Program Development and Coordination (Delivery of Quality Service)

1.As a mandated reporter, ensure all allegations of abuse and neglect are reported as required.

2. Ensure compliance with all HIPAA and confidentiality requirements.

3. Revises and monitors tracking system for all residential person-specific medical needs and appointments/screenings and assures 100% departmental compliance per Rule 115/116. Assists nursing staff with any coordination support that is needed.

4. Ensure documentation and records are maintained in accordance with pertinent policies, procedures, and standards of practice and complete steps to ensure that each person’s chart is up to date and accurate.

5. Continuously seeks feedback to ensure that medical support meets the needs of individuals served. This information gathering/reporting includes, but is not limited to, clinics and community providers and obtaining updates on hospitalized individuals at least every other day (every day for high severity/acuity hospitalizations).

6. As needed, schedule auxiliary medical services to include but not limited to internal primary physician, dental, podiatry, and vision clinics. Maintains partnerships with medical providers.

7. Lead and coordinate key on-site clinics such as primary care and psych clinic, assuring strong documentation practices and follow-through.

8. Complete and submit the required documentation after providing support to include but not limited to T-Log, case note, tracking, GER, and email updates to team members.

9. Supports the team in finding resources for all medical needs within each person’s specific insurance.

10. Attends team meetings as needed for medical concerns and assuring agency cross-collaboration. This may be person specific or site specific.

11. Oversees daily medication shipment check-in and notification of the RN-T of medication changes.
