Job Description:
We are seeking proactive, tech-savvy Female Medical Doctors with strong American-accent English proficiency to join our Chronic Care Management (CCM) team supporting U.S.-based healthcare providers. This is a non-prescribing, care coordination role focused on patient engagement, adherence monitoring, and digital health support for chronic disease patients across the United States.
Role Overview
This onsite position involves structured telemedicine interactions, digital health documentation, and remote patient monitoring. The ideal candidate is organized, empathetic, and passionate about proactive care delivery. You’ll play a vital role in bridging communication between patients and providers, ensuring continuity of care and adherence to treatment plans.
Key Responsibilities
Chronic Care Management (CCM)
- Conduct at least 75 structured calls/day on average to U.S. patients for enrollment and ongoing care coordination
- Discuss health concerns, medication adherence, and lifestyle goals
- Document all interactions in Electronic Health Records (EHR) for provider visibility
- Educate patients on preventive care and chronic disease management strategies
- Identify and escalate cases requiring provider intervention
Care Coordination & Communication
- Act as a liaison between our patients and our U.S.-based providers to ensure seamless care delivery
- Monitor patient progress and flag health concerns for care plan adjustments
- Maintain consistent follow-ups to ensure treatment adherence and engagement
- Support outreach initiatives to boost CCM program participation
Remote Patient Monitoring (RPM)
- Assist patients in using connected health devices to track vitals
- Interpret incoming health data and support timely medical interventions
Collaboration & Operational Support
- Work closely with U.S. care teams to optimize remote care workflows
- Contribute to digital health strategy improvements and outreach model enhancements
- Assist in hiring, training, and onboarding new staff
- Use Google Sheets to track performance metrics and patient progress
- Create structured notes and coordinate with billing teams to ensure insurance claims are processed accurately
Qualifications & Requirements
- MBBS or equivalent medical degree (required)
- Strong command of American-accent English (spoken and written)
- Prior experience in telemedicine, care coordination, or U.S. healthcare systems (preferred)
- Proficiency with EHR systems, Google Sheets, and digital health platforms
- Comfortable working night shifts and making high-volume patient calls

Pay: Rs80,000.00 - Rs200,000.00 per month
