Job Roles/Responsibilities:

Provide quality patient care and diagnostics to pregnant women at the field office including enrolment of patients, serial evaluations including examinations and diagnostics, data collection, maintaining records, and provide counselling.

  • Provide safe, respectful, and patient-centered primary healthcare services in line with approved clinical protocols;
  • conduct basic clinical assessment, triage, and referral for patients requiring higher-level care;
  • ensure infection prevention and control (IPC) practices are followed (hand hygiene, safe waste disposal, safe injection practices);
  • maintain confidentiality, ethical conduct, and patient dignity at all times;
  • deliver routine immunization services as per the national EPI schedule for eligible children and other target groups;
  • screen every eligible child visiting the facility for missed vaccinations and counsel caregivers to complete schedules;
  • ensure vaccine administration is safe and correct (right vaccine, dose, route, site, and documentation);
  • support defaulter tracing and follow-up of missed children through community linkages and outreach planning;
  • identify and promptly report Adverse Events Following Immunization (AEFI) as per national guidelines and support immediate management/referral when required;
  • coordinate with vaccinators, LHWs, community mobilizers, and UC staff to improve immunization demand and uptake;
  • deliver routine immunization services as per the national EPI schedule for eligible children and other target groups;
  • screen every eligible child visiting the facility for missed vaccinations and counsel caregivers to complete schedules;
  • ensure vaccine administration is safe and correct (right vaccine, dose, route, site, and documentation);
  • support defaulter tracing and follow-up of missed children through community linkages and outreach planning;
  • identify and promptly report Adverse Events Following Immunization (AEFI) as per national guidelines and support immediate management/referral when required;
  • coordinate with vaccinators, LHWs, community mobilizers, and UC staff to improve immunization demand and uptake;
  • plan, support, and monitor outreach immunization sessions in the Union Council to improve coverage in hard-to-reach and underserved areas;
  • ensure sessions are conducted as scheduled, with proper microplanning and community mobilization;
  • check readiness for outreach: vaccine availability, cold chain arrangements, safety boxes, registers/forms, and staff presence;
    supervise session quality: correct screening, safe injection, counseling, and accurate record-keeping;
  • document session performance (coverage, missed children, stock used, issues) and provide feedback to the team;
  • monitor daily activities at the health facility including staff attendance, patient flow, service continuity, and basic administrative functioning;
  • ensure essential supplies, vaccines, and consumables are available and used appropriately;
  • identify operational gaps (staffing, logistics, patient backlog, reporting issues) and escalate to the supervisor/program manager with practical solutions;
  • participate in regular meetings and coordination with UC and district health teams;
  • monitor daily activities at the health facility including staff attendance, patient flow, service continuity, and basic administrative functioning;
  • ensure essential supplies, vaccines, and consumables are available and used appropriately;
  • identify operational gaps (staffing, logistics, patient backlog, reporting issues) and escalate to the supervisor/program manager with practical solutions;
  • participate in regular meetings and coordination with UC and district health teams;
  • maintain complete, accurate, and timely documentation of services provided (OPD, immunization registers, tally sheets, stock records, referral slips);
  • compile and organize facility data regularly (daily/weekly/monthly summaries) for reporting to the program and relevant government systems;
  • monitor key indicators (coverage, dropouts, defaulters, session implementation, vaccine utilization) and flag data trends or inconsistencies;
  • ensure data quality through cross-checking registers, tallies, and summary reports before submission;
  • actively support government health campaigns (e.g., Polio, MR, outreach drives, SIAs) as assigned;
  • assist in community mobilization, team coordination, session monitoring, and campaign reporting;
  • ensure campaign activities align with government SOPs and maintain documentation required for verification;
  • provide counseling and health education to patients/caregivers on;
  • maintain coordination with facility staff, EPI staff, UC administration, community representatives, and program teams;
  • share updates, challenges, and achievements in a structured way through routine reporting and meetings;
  • follow organizational policies, ethics, safeguarding standards, and government health protocols;
  • consistent quality primary healthcare services delivered at the facility;
  • improved routine immunization coverage and reduced dropout/defaulter rates in the assigned UC;
  • outreach sessions conducted regularly and monitored for quality and completeness;
  • accurate, timely facility and immunization data compiled and submitted;
  • active participation in government campaigns with complete documentation and compliance.

Requirements:

  • MBBS;
  • additional qualification at clinical side would be preferred;
  • at least two years of experience;
  • strong knowledge of primary healthcare service delivery (basic assessment, triage, referral);
  • sound understanding of Routine Immunization / EPI schedule, eligibility, and safe administration practices;
  • knowledge of AEFI identification, immediate response, and reporting procedures;
  • understanding of cold chain basics and vaccine handling/safety requirements;
  • familiarity with Government campaign operations (Polio, MR, SIAs) and compliance with SOPs;
  • ability to conduct supportive supervision and monitoring of facility and outreach session activities;
  • strong skills in data recording, compilation, cleaning, and reporting (registers, tally sheets, monthly summaries);
  • good data quality checking ability (cross-verification between registers, tallies, and reports);
  • ability to track and act on defaulters/dropouts through follow-up coordination and community linkages;
  • strong communication and counseling skills for caregiver education and addressing vaccine hesitancy;
  • competency in community engagement and mobilization for outreach and campaign support;
  • working knowledge of infection prevention and control (IPC) standards and safe waste disposal;
  • good coordination skills with UC staff, EPI teams, facility staff, and district/program teams;
  • ability to identify gaps, problem-solve, and escalate issues appropriately with practical solutions;
  • strong traits: integrity, professionalism, respect for confidentiality, accountability, and teamwork;
  • ability to work under pressure during campaigns with time management and reliability;
  • basic computer literacy: Excel/Word/data entry for reporting and record maintenance.
