Medical Social Worker

Job Summary
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. Duties

• Conduct comprehensive patient assessments to identify social, emotional, and behavioral needs, including diagnostic evaluation and mental health counseling.
• Provide individual, group, and family therapy utilizing evidence-based practices such as behavioral therapy, cognitive behavioral therapy, motivational interviewing, and applied behavior analysis.
• Facilitate crisis intervention and manage complex cases involving hospice care, Alzheimer’s care, PTSD care, dementia care, or working with individuals with developmental disabilities or autism.
• Collaborate with healthcare providers to develop and implement personalized care plans that incorporate social work principles and case management strategies.
• Assist patients with discharge planning, home & community care coordination, assisted living placement, and post-acute care arrangements.
• Maintain accurate medical documentation in compliance with HIPAA regulations using Electronic Medical Records (EMR) systems; ensure timely data collection and record keeping.
• Support research initiatives related to behavioral health or clinical research projects; utilize ICD-9/ICD-10 coding for documentation purposes.
• Engage in intake procedures for new patients and provide ongoing clinical counseling tailored to individual needs across outpatient or inpatient settings.
• Participate in interdisciplinary team meetings to discuss patient progress and adjust treatment plans accordingly.

Skills

• Strong background in social work with expertise in mental health counseling, behavioral health, crisis management, and case management.
• Proficiency in medical terminology, medical records management, CPT coding, ICD coding (ICD-9/10), and HIPAA compliance standards.
• Experience working with diverse populations including children (special education), seniors (geriatrics), individuals with disabilities or developmental disabilities, and those requiring hospice or post-acute care services.
• Skilled in various therapeutic modalities such as cognitive behavioral therapy (CBT), behavioral therapy, motivational interviewing, group therapy, and psychotherapy techniques.
• Familiarity with EMR systems for efficient documentation; ability to collect data accurately for clinical research or quality improvement projects.
• Excellent communication skills for intake interviews, patient assessment discussions, family counseling sessions, and interdisciplinary collaboration.
• Ability to handle crisis situations effectively through crisis intervention strategies; experience working in inpatient settings or nursing homes is preferred.
• Knowledge of working with individuals with autism spectrum disorder (ASD), Alzheimer’s disease, PTSD or dementia; experience in hospice or senior care settings is advantageous.
