Job Summary
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. In this vital role, you will provide psychosocial support, counseling, and resource coordination to patients and their families across various clinical settings. Duties
- Conduct comprehensive patient assessments to identify social, emotional, and behavioral needs, including diagnostic evaluation and mental health counseling.
- Provide individual, group, and family therapy utilizing evidence-based practices such as behavioral therapy, cognitive behavioral therapy, motivational interviewing, and applied behavior analysis.
- Facilitate crisis intervention and manage complex cases involving hospice care, Alzheimer’s care, PTSD care, dementia care, or working with individuals with developmental disabilities or autism.
- Collaborate with healthcare providers to develop and implement personalized care plans that incorporate social work principles and case management strategies.
- Assist patients with discharge planning, home & community care coordination, assisted living placement, and post-acute care arrangements.
- Maintain accurate medical documentation in compliance with HIPAA regulations using Electronic Medical Records (EMR) systems; ensure timely data collection and record keeping.
- Support research initiatives related to behavioral health or clinical research projects; utilize ICD-9/ICD-10 coding for documentation purposes.
- Engage in intake procedures for new patients and provide ongoing clinical counseling tailored to individual needs across outpatient or inpatient settings.
- Participate in interdisciplinary team meetings to discuss patient progress and adjust treatment plans accordingly.
Skills
- Strong background in social work with expertise in mental health counseling, behavioral health, crisis management, and case management.
- Proficiency in medical terminology, medical records management, CPT coding, ICD coding (ICD-9/10), and HIPAA compliance standards.
- Experience working with diverse populations including children (special education), seniors (geriatrics), individuals with disabilities or developmental disabilities, and those requiring hospice or post-acute care services.
- Skilled in various therapeutic modalities such as cognitive behavioral therapy (CBT), behavioral therapy, motivational interviewing, group therapy, and psychotherapy techniques.
- Familiarity with EMR systems for efficient documentation; ability to collect data accurately for clinical research or quality improvement projects.
- Excellent communication skills for intake interviews, patient assessment discussions, family counseling sessions, and interdisciplinary collaboration.
- Ability to handle crisis situations effectively through crisis intervention strategies; experience working in inpatient settings or nursing homes is preferred.
- Knowledge of working with individuals with autism spectrum disorder (ASD), Alzheimer’s disease, PTSD or dementia; experience in hospice or senior care settings is advantageous.
